Location: James Street Family Practice / Tasburgh Lodge Surgery

Hours:Full time (37.5hrs)

Salary:Negotiable depending on experience.

Job Purpose

The Care Coordinator will provide both administrative and clinical support within the Primary Care team, helping to deliver safe, effective, and patient-centred care. The role will support clinicians in managing patient care, coordinating services, and undertaking healthcare clinical duties within agreed competencies and training.

The post holder will work closely with GPs, nurses, healthcare assistants, pharmacists, and the wider multidisciplinary team to improve patient access, continuity of care, and health outcomes.

Main duties of the job

Main duties of the job

As a Care Coordinator, you will support patients through their care journey, ensuring they receive the right care, at the right time, with the right clinician. You will work closely with the clinical and administrative teams to coordinate care, manage information, and support practice processes.

You will be a key point of contact for patients in person, by phone and online. Helping them understand next steps, navigate services and complete required actions.

Key responsibilities

Care navigation using practice protocols to direct patients to the right clinician or health care service. Coordinating patient care. Accurate coding of information into patient records. Managing patient communication via phone, online requests and face-to-face contact. Supporting workflow including scanning, coding, tasks and administrative processes Liaising with clinical teams to ensure actions are completed. Booking appointments and contacting patients where required. Maintaining confidentiality and following practice procedures at all times. Providing general support to GPs, management team and wider practice team.

Key Responsibilities

Clinical Duties

Working within competency, training, and practice protocols, the Care Coordinator will undertake healthcare assistant-style clinical duties including:

Recording baseline clinical observations. Performing venepuncture / phlebotomy, ECG recordings, Urinalysis, simple wound care and dressings, NHS health checks Supporting chronic disease monitoring clinics. Assisting with vaccination clinics, infections prevention and control procedures, stock monitoring and clinical room preparation. Supporting clinicians during minor surgical procedures Identifying and escalating abnormal findings or safeguarding concerns appropriately. Maintaining accurate clinical records on practice systems.

Administrative Responsibilities

Maintain accurate patient records using SystmOne. Process referrals, correspondence, and clinical documentation. Support with the management of insurance reports and incoming letters. Support the practice with achieving QOF targets Assist with data collection, audits, and quality improvement work. Manage patient communications via telephone and digital systems. Ensure confidentiality and compliance with GDPR and practice policies.

Multidisciplinary Team Support

Work collaboratively with GPs, nurses, HCAs, pharmacists, and wider MDT members. Participate in practice meetings and service development initiatives. Support proactive patient care and preventative health programmes. Promote a positive patient experience and continuity of care.

The post holder will: Work within defined competencies and training. Undertake mandatory and role-specific training. Adhere to all clinical governance, safeguarding, infection control, and data protection policies. Promote equality, diversity, and inclusion at all times.

Experience Essential

Essential Criteria: Experience working in a healthcare or primary care setting. Experience undertaking clinical observations and basic clinical procedures. Good communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Strong organisational and administrative skills. Knowledge of confidentiality, safeguarding, and infection control.

Desirable

Desirable Criteria: Experience in General Practice or Primary Care. Competency in phlebotomy and ECG recording. Knowledge of long-term condition management. Familiarity with SystmOne. NVQ Level 3 in Health & Social Care or equivalent. Care coordination or HCA experience.
