About this role

Job summary

You will be part of The Luton Community Mental Health Services. Our team is part of the Mental Health Transformation services where the blended team (voluntary service colleagues) are fully embedded within the team.

Main duties of the job

You will work as an autonomous practitioner within a dedicated Multidisciplinary Hub. If you are committed, relish new challenges and strives for excellence, this post is for you.

You will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the Operational Policy.

This will involve the assessment, care planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the clients. The role includes being part of a duty rota, screening referrals and participating in Quality Improvement (QI) projects within the Team.

You will also take part in the Transformation Agenda in line with The NHS Long Term Plan, which describes:

New community-based offer which will include access to psychological therapies, improved physical health care, employment support, personalised and trauma-informed care, medicines management and support for self-harm and co-existing substance use and proactive work to address racial disparities.

To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities To promote a service user focused service Participate in audits and survey relevant to the team To participate in development of new policies and procedures as required Ensure that clients receive a culturally competent service To co-ordinate the day-to-day activity and manage the workload of the Team To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments are adhered to To establish and maintain good relationships with service users, carers and voluntary sector colleagues To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines. Liaise with General Practice and Primary Care colleagues to promote partnership working Ensuring that the local demographics are taken into consideration for service provision and improvement

Person Specification

Education/ Qualification/ Training Essential

Registered Nurse (Mental/ Learning Disability Nurse) with the UK NMC

Desirable

qualifying experience in a community mental health team

Training/Experience Essential

post qualification Experiance MSPP qualification or equivalent

Experience of working in a community mental health team Evidence of further training

Knowledge and Skills Essential

Assessment skills Physical health assessment and monitoring Administering of depot medication Medication Management Good understanding of mental health conditions and treatment options in line with NICE guidance Presenting cases and chairing meetings Risk assessment and relapse prevention work establishing good relationships with good communication skills with SU's/carers/MDT/partner organisations
