Job summary

The States of Guernsey are looking for a Support Worker to join the Learning Disability Outreach Team.

The role supports adults with multiple learning disabilities and Autism Spectrum Disorder to live meaningful, independent lives in the community.

The team provide community-based work carried out across the island. Staff apply person centred practice and use the Active Support Model, focusing on doing tasks with individuals to build skills, confidence, and choice.

Responsibilities include assisting with daily living activities such as personal care, medication, cooking, cleaning, shopping, finances, and community access while promoting dignity, participation, and wellbeing.

All support is delivered with compassion and consistency, reflecting Guernseys Care Values. Band 3 staff are required to work independently and alongside colleagues under the guidance of senior staff to provide high quality physical and emotional care, and they act as co-key workers in line with service values and quality standards.

A flexible shift pattern is required to meet the needs of service users within their individual care packages. Lone working will be required.

Job description Job responsibilities

To support patients/service users in undertaking activities of daily living within care plans.

To monitor and report changes in patient/service users presentation to the staff member in charge and take appropriate action.

To follow all relevant policies and procedures.

To report issues of concern in line with policy and contribute to any Quality and Safety processes.

Person Specification

Qualifications Essential

Ability to demonstrate an understanding of HSCs CARE Values Punctual and reliable Ability to follow instructions Ability to organise own work Ability to work well within a team Self-motivated and able to use initiative Good level of literacy and numeracy Good communication skills (written and verbal) Driving licence The postholder will be supported to achieve either of the following within an agreed timeframe. Care Certificate within probation period Band 3 Competency Pathway Candidates who hold a Level 3 VQ Diploma in Health & Social Care will be deemed to have an equivalent qualification and may not need to complete the full Band 3 Competency Pathway.
