Main duties of the job As a PCN Prescribing Pharmacist at Operose Health, you will be an essential member of the practice team, collaborating closely with clinicians to enhance patient care and make cost-effective prescribing decisions. Key responsibilities include running daily telephone triage clinics, managing patient assessments where necessary, reviewing pathology results, and liaising with local pharmacies to optimize patient outcomes. The role involves managing repeat prescription services and dealing with queries from both reception staff and patients. You will also participate in home visits and immunization initiatives, all while improving the quality of primary care services. The ideal candidate must be GPhC registered, possess excellent communication skills, and show a strong commitment to improving patient care. Main Responsibilities: As a valued member of the Operose Health team, your key duties will include:

As a member of the practice team, you will work closely with our clinicians to help and improve patient care and make cost effective proscribing decisions. You will run daily telephone triage clinics and call in patients for assessment and management where clinically indicated, as well as review and triage pathology results, contacting patients. Provide practice-based review clinical to those with LYCs or complex health needs. Review and action incoming hospital correspondence, particularly with regards to medicine reconciliation or initiation, liaising with local pharmacies to improve patient outcomes. Lead practice repeat prescription service and deal with queries from reception staff and patients. Also participate in home visits and immunisations initiatives.Ideal Candidate: What we look for in new colleagues:

GPhC registered. Excellent communication skills both verbally and written as well as the ability to work well as a part of a team and willingness to develop clinically. A can-do attitude with a real desire to improve patient care. Package Description: How well support you:

Annual salary of up to £45,000 depending on qualifications and experience. 27 days annual leave plus bank holidays pro rata.
