Job summary

This is a fixed term post until 31st March 2027.

The Afan cluster is recruiting a children and young people's well being coordinator. Currently services to support young people with their mental health are available across health, education, local authority and the third sector. However the system is dis-jointed which makes it hard for front line health practitioners to discuss options with young people and set them on a path which gives them the best chance of getting the support they need.

This post offers the opportunity to map this pathway, develop information for health practitioners and use relationships, knowledge and data to co-design sustainable models for the future. You will also have the chance to work directly with young people and have a positive impact on their health.

Main duties of the job

Principal Responsibilities

The Childrens Wellbeing Coordinator will play a central role in developing and delivering a more joined-up approach to supporting children and young people. The postholder will:

Undertake a detailed mapping exercise of current services that support children and young peoples mental health and emotional wellbeing across education, health, third sector, and statutory servicescapturing referral criteria, capacity, and waiting times.

Develop and maintain accessible, up-to-date resources to support decision-making and enable timely, appropriate referrals or sign posting.

Work directly with young people and families to discuss next steps and refer/signpost to services that can support.

Act as a key point of contact for health professionals seeking advice and guidance on available services and pathways.

Use insight and evidence to inform continuous improvement and make recommendations for system change and pathway refinement, monitor the effectiveness and impact of change on patients and relatives accordingly.

Work collaboratively with service users and health professionals in the local community to build a clear picture of local need by collating and analysing data; identifying gaps in provision and making recommendations for service development and a sustainable model moving forwards.

About us

The post holder will be working for the Afan Cluster and hosted by Mount Surgery. You will need a flexible approach to work and will be based across different GP practices and offices within the Afan cluster at different times to meet service need.

Management and supervision will come from a range of sources:

Line management will sit within Mount Surgery.

Clinical supervision and escalation with be with the community psychology team.

Daily activity in terms of performance, goal setting and project management/implementation will sit with the cluster lead and lead GP for the cluster

In partnership with Trailblazers, the Afan cluster is looking to recruit a Childrens Well Being Coordinator.

Currently there is substantial provision available to support young people with their mental health across a range of sectors including health, education, 3rd sector and local authority provision. However the overall system isnt joined up with a lack of information available around what different organisations provide, what capacity they have and how to refer etc. This makes it very difficult to ensure that young people are directed to the best service to meet their needs, and can lead to delays in accessingappropriate support, duplication of referrals, and families feeling uncertain or unsupported as they are moved between services.

The post holder for this role will spend time working across the system to build relationships and capture the breadth of support available. This information will be used to:

1 - Develop and provide resources and expert guidance to support primary care staff in ensuring children and young people are directed to the right service at the right time, and that families receivetimely,appropriate, and joined-up support.

2 - Support young people identified through the trailblazer initiative (read more here) in accessing appropriate services.

Person Specification

Additional Criteria Essential

Ability to work autonomously and use initiative to drive work forward. Strong interpersonal skills, with the ability to build effective relationships across organisations Confidence in facilitating discussions, including appropriately challenging where needed in a constructive and evidence-based way Ability to analyse, interpret, and present complex information or data clearly Demonstrable commitment to improving outcomes for children and young people Willingness and ability to travel across the geographical area Innovative and solution-focused approach Self-motivated, proactive, and able to work using own initiative

Qualifications Essential

A recognised qualification in a field related to children and young people (e.g. education, youth work, health, social care, or therapeutic disciplines), or equivalent professional experience. Masters level qualification or equivalent experience.

Desirable

Qualification or training in quality improvement and/or project management

Experience Essential

Significant experience of working with children and young people, including supporting those with mental health, emotional, or behavioural challenges Experience of working within multi-disciplinary and multi-agency environments Experience of coordinating services, projects, or care pathways Knowledge of the systems and services supporting children and young people across health, education, social care, and the third sector Understanding of current challenges and opportunities within children and young peoples mental health services Experience of using data, research, or service mapping to inform practice or service development Experience of service mapping, pathway design, or service redesign Knowledge of the children and young peoples mental health system in Wales

Desirable

Working on evidence based whole system change projects in healthcare.
