Main duties of the job
As a Clinical Pharmacist within our Practice, you will:
Conduct structured medication reviews (SMRs) for patients with complex needs. Manage long-term conditions such as hypertension, asthma, and diabetes. Provide expert advice on medicines optimization and safety. Support repeat prescription processes and medication reconciliation. Collaborate with GPs, nurses, and other healthcare professionals to deliver integrated care. Manage safety alerts/Risk stratification /audits Care home medication reviews Medicines quality improvement and service development

Job purpose
we are looking for a motivated and enthusiastic clinical pharmacist to join our friendly and forward thinking team at shipdham surgery as part of the mid norfolk primary care network this is an exciting opportunity to play a key role in delivering high quality patient care within a supportive multidisciplinary team
the post holder will work within their clinical competencies to improve the quality safety and effectiveness of care for patients you will work closely with gp partners nurses and the wider team to support safe prescribing optimise medicines use and help manage patient demand across the practice
Main duties and responsibilities
provide patient facing medication reviews with a focus on long term conditions polypharmacy and frailty
manage prescription requests and make clinical decisions including prescribing where appropriate
support patients with medicines queries and provide advice on minor ailments
work with care homes to improve medicines safety and carry out medication reviews
support safe prescribing through audit medicines optimisation and quality improvement work
assist with hospital discharge medicines reconciliation and reduce medicines related harm
contribute to antibiotic stewardship and infection prevention initiatives
work collaboratively with community pharmacy hospital teams and other healthcare professionals
support the development of efficient systems for prescribing and medicines management within the practice
communication and teamwork
build strong working relationships within the practice and across the wider healthcare system
communicate effectively with patients colleagues and external organisations
work as part of a team recognising when to seek support from senior clinicians
delivering a quality service
organise and prioritise workload effectively
maintain up to date clinical knowledge and professional registration

Person Specification
Qualifications Essential
Essential Completion of an undergraduate degree in pharmacy. Working towards an independent prescribing qualification Primary Care Pharmacy Education Patchway (PCPEP) qualification Demonstrates understanding of and conforms to, relevant standards of practice Follows professional and organisational policies/procedures relating to performance management Registered with the General Pharmaceutical Council (GPhC). Primary Care Pharmacy Education Patchway (PCPEP) qualification Essential Minimum of 2 years post qualifying experience as a pharmacist.
Desirable
Independent Prescriber Qualified Desirable Experience in primary care setting Strong communication and interpersonal skills. Commitment to continuous professional development.
