Job summary
You will be part of the respective specialty teams on an acute specialty/GIM medicine wards based on Rookwood A and B (Elderly care), Hazelwood ward (Respiratory), Brindle (Gastroenterology and Elderly care), Coronary care Unit and Medical assessment unit. You will work with the multidisciplinary team to provide excellent care to our patients.
We will provide you with excellent clinical training in Geriatric medicine / Respiratory /Gastroenterology / Cardiology and Acute medicine during your rotation. You will be able to gain experience and knowledge in the above specialties. We are keen that you are afforded all the educational opportunities of specialist trainees in each of the posts. You will be encouraged to attendappropriate localtraining sessions and complete formal assessments in line with what is expected of junior trainees. At the end of the post, you will therefore be able todemonstrateevidence of learning and the acquisition of skills and competences that will strengthen any applications for future training posts.
The post holder will rotate every 4 months between Elderly care/GIM x 2, Respiratory/GIM x 1, Cardiology/GIM x 1, Gastroenterology/GIM x 1 and Acute medicine x 1 specialties and will be based at Chorley DGH.
The post holder willparticipatein an unselected General Internal Medicine on call on a one in 14 rota. The on call includes night shifts and weekend work.

2 years initially.
Main duties of the job
Be involved directly with the day-to-day functioning of your respective wards, and any associated medical outlying patients.
Participate in multidisciplinary team working (including physiotherapy, occupational therapy, speech and language therapy, mental healthliaisonand palliative care etc.) to promote patient independence and wellbeing, and proactive discharge planning.
Participate in consultant- and registrar-led wardrounds, andalso perform independent ward rounds under supervision.
Undertake the administrative duties involved with the care of your patients.
Perform procedures (venepuncture, cannulation, arterial blood gas sampling, NG tube placement, catheters, mental state examination,Asitictap)
Develop communication skills (including obtaining collateral histories, conducting familymeetingsand addressing advance care planning needs)
Promote high professional standards of clinical care for patients, both through direct patient care and by the maintenance of continuing professional development.
Maintain continuing education and professional development.
Activelyparticipatein the audit process and the quality improvement activity in the department.

This rotational post is based on Rookwood A&B wards for Elderly medicine, Brindle ward for Gastroenterology and GIM, Hazelwood ward for Respiratory medicine, Coronary care for Cardiology and Medical assessment unit for Acute medicine based at Chorley Hospital. The bed numbers vary between 24 to 30 on these wards.Each ward will be looked after by the respective Consultant teams and will have across coverarrangement.
