We are seeking a motivated and patient-focused Prescribing Advanced Nurse Practitioner to join our clinical team. The successful candidate will play a key role in delivering high-quality clinical care, supporting safe and effective prescribing, and improving patient outcomes across a range of clinical services.

The Prescribing Nurse Practitioner will work collaboratively with GPs, nurses, and multidisciplinary healthcare professionals to assess patients, manage long-term conditions, conduct medication reviews, and prescribe within their scope of practice. This role requires strong clinical knowledge, excellent communication skills, and a commitment to delivering patient-centred care.

Main duties of the job

Conduct patient consultations and clinical assessments

Prescribe medications safely and appropriately as an Independent Prescriber

Perform structured medication reviews and medicines optimisation

Support management of chronic diseases such as hypertension, diabetes, asthma, and COPD

Monitor treatment effectiveness and manage side effects or medication changes

Provide advice and education to patients regarding medicines and treatment plans

Work collaboratively within a multidisciplinary healthcare team

Maintain accurate clinical records and ensure compliance with professional standards

Contribute to service development and quality improvement initiatives

Job description Job responsibilities

We are looking for an experienced and patient-centred Prescribing Advanced Nurse Practitioner to join our healthcare team. The successful candidate will be responsible for delivering high-quality clinical pharmacy services, supporting safe and effective prescribing, and improving patient outcomes through medicines optimisation and evidence-based care.

The Prescribing Advanced Nurse Practitioner will work collaboratively with GPs, nurses, and other healthcare professionals to assess, treat, and manage patients within their scope of practice while ensuring compliance with all professional and regulatory standards.

Key Responsibilities

Conduct patient consultations, clinical assessments, and medication reviews

Prescribe medications safely and appropriately as an Independent Prescriber

Support the management of long-term conditions including hypertension, diabetes, asthma, COPD, and cardiovascular disease

Review and optimise patient medications to improve safety, adherence, and clinical outcomes

Monitor treatment effectiveness and adjust therapy where clinically appropriate

Identify and reduce prescribing risks, polypharmacy issues, and medication errors

Provide patients with advice and education regarding medicines and treatment plans

Maintain accurate and up-to-date clinical records

Work collaboratively within a multidisciplinary healthcare team

Contribute to clinical audits, quality improvement initiatives, and service development

Ensure compliance with NHS guidelines, local policies, and professional standards

Participate in continuing professional development and mandatory training

Person Specification

Experience Essential

Experience of working within a nursing team Experience of Independent Prescribing Experience of dealing with vulnerable patients

Desirable

Experience of working in a GP practice

Qualifications Essential

Registered Nurse with current registration with the Nursing and Midwifery Council (NMC) Advanced Clinical Practice qualification Independent Prescriber Experience managing minor illness and acute presentations in primary or urgent care Strong clinical assessment and decision making skills Ability to work autonomously in a fast-paced environment Excellent communication and patient care skills Competence in history taking, clinical assessment, and decision-making

Desirable

Postgraduate modules in minor illness / minor injury management Experience using EMIS Qualification or training in long-term condition management (e.g. diabetes, asthma, COPD) Experience or certification in primary care / GP surgery
