Registered Social Worker
BayWest Medical Centre ? Ajax, Ontario
BayWest Medical Centre is seeking a *Registered Social Worker* to join our growing healthcare team. The successful candidate will provide compassionate, patient-centred support to individuals and families experiencing a range of social, emotional, and life challenges.
Position responsibilities
- Conduct psychosocial assessments and identify patients? social and emotional needs
- Provide individual counselling and supportive interventions
- Support patients experiencing stress, anxiety, grief, family concerns, and other psychosocial challenges
- Assist patients with navigating community resources and social services
- Provide support with housing, financial, employment, and other social needs where appropriate
- Develop individualized care plans and provide appropriate follow-up
- Collaborate with physicians and other healthcare professionals as part of a multidisciplinary team
- Provide crisis support and appropriate referrals when required
- Maintain accurate and timely documentation in the EMR
- Maintain patient confidentiality and adhere to professional and ethical standards
Qualifications
- Registered Social Worker in good standing with the Ontario regulatory body
- Bachelor?s or Master?s degree in Social Work
- Previous experience in a healthcare, primary care, or community setting is an asset
- Strong counselling, communication, and interpersonal skills
- Ability to work independently and collaboratively within a multidisciplinary healthcare team
- Experience with electronic medical records (EMR) is an asset
- Compassionate, professional, reliable, and patient-focused
