About this role

Job summary

The post holder will provide a comprehensive service to the Trust with a focus to facilitate effective and timely discharge of patients no longer requiring acute care. The post holder will provide a single point of contact for the management of bed availability and allocation within ward. The role requires a consistent and coordinated approach to the operational management of patient flow, with an emphasis on maintaining an on-going and 'real time' understanding of the ward's current and predicted capacity position and the timely and well planned discharge process.

The post holder is required to work collaboratively with the multidisciplinary team internal and external to the organisation

***Working shift pattern as follows - 07:00am-19:15pm, 13:00pm-21:00pm and 19:00pm-07:15am***

Main duties of the job

To support the ward manager and ward co-ordinator in maintaining patient flow

To manage the maintenance of e-track to ensure it reflects the current

Discharge plans and the ward bed availability

Liaise with the duty manager when a bed is available.

Attend Whiteboard rounds, identify actions required and actively managed agreed discharge plans for patients with predicted discharge dates within the next 3 days.

Maintain whiteboards and e-track information to ensure it is up to date.

Arrange or where necessary photocopy notes for patient transfer.

Assist in the completion of discharge check lists to ensure accurate discharge planning is evident.

Chase doctors for completion of discharge letter and deliver TTOs if necessary.

Ensure diagnostic requests are expedited promptly and escalate any delays to the appropriate person.

Ensure plans are agreed with patient and carers for discharge and that they are fully aware of any requirements to assist discharge.

Liaise with the Transfer of Care teams and other clinical specialties to reduce delays.

Utilise IT systems to secure and record timely and accurate data relevant to the role and function.

Coordinate admissions and transfers in the wards, and external transfers to other organisations.

Very high levels of organisational skills are required to fulfil this job satisfactorily. The ward is very busy and the post holder will know how to multi task and change priorities quickly.

The post holder must understand how to plan and prioritise the workload on their own initiative.

The post holder is required to think ahead and make judgement as to what order task should be completed.

Work cooperatively as part of a multidisciplinary team.

This post-holder works within the ward area and is required to deal with constant interruptions in concentration and changing priorities

The post-holder requires a calm, logical mind at all times

To be able to work calmly under pressure in a busy environment and have the ability to respond to the changing work pace in the department

The post holder must concentrate on all aspects of their role at all times. Each function of their role requires accuracy and speed to ensure patients are discharged in a timely fashion.

Ensure data is captured accurately and recorded on the hospital computer systems.

Ensure that staff is aware of the necessity for complete confidentiality related to their work and to encourage them t seek advice if in doubt.

Work under considerable pressure due to the volume of both external internal users of the service, covering a workload of multiple specialties, carried out across the Trust.

Person Specification

Education and Qualifications Essential

Good standard of literacy and numeracy NVQ Level 3 or equivalent experience

Desirable

Junior Management/Team Leader Qualification

Experience Essential

Previous Customer Service experience (minimum of 12 months) 1 year previous administration experience to NVQ level 3 or equivalent

Desirable

1 year experience of working in an ED / healthcare environment Project/change management skills to facilitate service improvement

Knowledge Essential

Clear, concise verbal and written communication

Desirable

Experience in customer service and handling sensitive information. Experience in providing admin support related to patient care

Other Essential

Use to working in a busy environment Demonstrates a strong desire to improve performance and make a difference by focusing on goals
