About this role
Job summary You'll support people at key moments in their recovery by helping them access the right rehabilitation and community support, promoting safe and timely care closer to home. This role plays a vital part in reducing unnecessary hospital stays and supporting smooth transfers of care. You'll work within a multidisciplinary team alongside health and social care partners. Using your clinical skills and judgement, you'll assess needs, agree next steps with patients and carers, and help plan the right level of support. You'll manage your own caseload while contributing to service improvement and shared learning.
Main duties of the job What you'll do:
Assess people with a range of health needs to identify rehabilitation and support options Make clear, evidence-based recommendations that support discharge and independence Work closely with colleagues across health and social care to coordinate care plans Manage a complex caseload, prioritising work safely and effectively Keep accurate records using electronic systems and support service audits What we're looking for: A registered healthcare professional with current NMC or HCPC registration Experience of assessing and managing patients, including complex presentations Confidence working autonomously and as part of a multidisciplinary team Clear communication skills with patients, carers and professionals A commitment to learning, reflective practice and high-quality care We welcome applications from people with different backgrounds and experiences and are committed to fair, inclusive recruitment where everyone is treated with respect and supported to thrive.

Able to justify clinical decisions to other Medical Staff, colleagues and other professionals Competent in skills appropriate to the post e.g. Carry out clinical assessment of patient need in line with intermediate care team requirements Evidence of CPD activity, including nationally recognised post graduate training in relevant speciality to Diploma level or equivalent Evidence of reflective practice, IST and CPD portfolio Autonomous practitioner works within codes of practice, policies and guidelines.
