Duties
Review, verify, check and dispense all prescribed medication orders for appropriatenessIdentify, resolve and prevent drug related problemsMonitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelinesConduct medication reconciliationFollow up on medication incidents and promote safe medication practices on the unitRespond to drug information questionsProviding direct and/or functional supervision of technician staffPerforming other duties consistent with the job classification, as required Completion of university degree (Bachelors of Science Degree in Pharmacy or Doctor of Pharmacy degree), or equivalent requiredCertificate of competency from the Pharmacy Examination Board of Canada requiredActive registration with the Ontario College of Pharmacists (Part A) requiredCertificate of competency from an accredited smoking cessation program and flu vaccination injection program requiredFamiliarity with common drug regimens and dispensing process for specialty medications, as well as familiarity with dispensing processes for out of province patientsExperience working as part of a multi-disciplinary teamStrong interpersonal, communication and customer service skillsWorking knowledge of adherence programs including but not limited to compliance packaging, refill synchronization, and refill managementManual billings, out of province billings and specialty meds related billingsExperience/working knowledge of HealthWatch Delta paperless workflow module and Points of Sales SystemExperience handling pharmacy inventoryExcellent verbal and written communication skillsExcellent customer service & interpersonal skillsExcellent organizational and time management skillsHospital Outpatient Pharmacy practice experience preferredHospital pharmacy residency an assetExperiences in handling blister pack an asset Why join UHN?
