Key Responsibilities RN Case Manager Responsibilities
• Complete comprehensive nursing assessments and develop individualized care plans • Provide skilled nursing interventions in patient homes • Manage a balanced caseload with strong organizational efficiency • Coordinate care with interdisciplinary team members • Educate patients and caregivers on disease processes, medications, and safety • Maintain accurate, timely documentation • Communicate effectively with physicians, families, and internal team members • Support safe transitions from hospitals, SNFs, and outpatient settings
Schedule Full‑Time Schedule
• Monday–Friday • Daytime schedule • Balanced caseload with strong team support • Limited on‑call rotation depending on team structure
Qualifications RN Requirements
• Current Washington RN license (or ability to obtain) • One year of nursing experience preferred; home‑health experience a plus • Strong assessment, communication, and critical‑thinking skills • Ability to work independently in a field‑based role • Reliable transportation and valid driver’s license
