Home In The Heart Healthcare Agency is seeking a compassionate and experienced Registered Nurse (RN) to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services.
Service Area: Hardin County and surrounding areas
Key Responsibilities
• Conduct initial assessments, supervisory visits, and reassessments.
• Evaluate client health, functional needs, safety, and home environment.
• Develop and update individualized care plans.
• Provide clinical guidance and oversight to caregivers.
• Communicate with clients, families, physicians, case managers, and VA representatives.
• Complete accurate and timely documentation.
• Ensure compliance with agency, VA, and Tennessee regulations.
Qualifications
• Current, unrestricted Tennessee RN License
• Valid driver’s license and reliable transportation
• Strong clinical assessment and documentation skills
• Excellent communication and organizational skills
• Ability to work independently and travel within the service area
Preferred: Experience in home health, geriatrics, veterans care, case management, long-term care, or community-based nursing.

• PRN / Flexible scheduling
• Visits scheduled based on client needs and referrals
• Competitive per-visit assessment rate
• Mileage reimbursement may be available per agency policy
