Are you a compassionate and dedicated Healthcare Assistant looking for a role where your care can make a meaningful difference every day? Join our community team in Lincolnshire and help provide personalised, dignified end-of-life care that supports people to remain at home, including in residential homes.
In this rewarding role, you will work as part of a supportive multidisciplinary team, helping people feel safe, comfortable and respected at a highly important time in their lives. You will support individual care plans by assisting with personal care, skin integrity, nutrition and hydration, mobility, rest and sleep, and maintaining a clean and safe care environment.
Main duties of the job
You will play an important role in monitoring each persons wellbeing, recognising and responding to changes in their physical or emotional condition. Working within the agreed care plan, you will support symptom management, communicate relevant observations and promptly escalate any concerns to registered healthcare professionals.
You will also provide calm, compassionate and reassuring support to patients and those important to them. By listening to individual needs and preferences, you will help ensure people feel heard, respected and supported, while promoting dignity, comfort, choice and wellbeing.

Contract: As & When
Shifts: Night shifts between 20:00 and 08:00, with the service operating 365 days a year
Salary: Marie Curie Pay Scale Band 2, aligned with AFC, at £12.92 per hour. Enhanced pay applies for weekend and bank holiday shifts, with uplifts of up to 41% for Saturdays and up to 83% for Sundays and bank holidays
Location: Community-based in Lincolnshire covering Boston, Grantham and Lincoln
What we are looking for:
Relevant recent experience providing care in a Health or Social Care Environment, including caring for patients at the end of life. NVQ Level 2 in Health and Social Care or equivalent qualification. Be person-centred and compassionate when caring for the person and those important to them. Ability to develop and evaluate care management plans that meet the complex needs of the person, and those important to them. Emotional resilience and the capacity to work independently. Access to a personal vehicle, a valid driving license and business insurance. You must be able to travel to the Lincoln, Boston and Grantham bases as required.What's in it for you:

Qualifications Essential
NVQ Level 2 in Health and Social Care or equivalent qualification Functional skills at level 2 in numeracy and literacy or equivalent

Recent care delivery experience in a Health or Social Care Environment
