Join UTHealth and lead a multidisciplinary diabetes education and nutrition team supporting the clinical practices of UT Physicians Obstetrical, Gynecological and Reproductive Sciences clinics. This leadership role oversees the implementation and evaluation of a comprehensive diabetes self-management education program while providing patient care through group visits, telemedicine, and in-person appointments. Position Summary: The Manager of Diabetes Education Program manages the implementation and evaluation of the comprehensive diabetes self-care management and education program. Delivers a collaborative multidisciplinary team infrastructure with the aim to enrich the patient and caregiver experience specific to people living with diabetes. Position Key Accountabilities:

• Oversees diabetes education program including creating and monitoring implementation and execution; setting goals for department, quality incentive programs and American Diabetes Association (ADA) recognition program; evaluating efforts for quality and practice improvement. • Participates in population health and care coordination activities that are sponsored by the Department that impact the continuum of care and provide chronic disease and health promotion expertise. • Brings to bear knowledge and experience in evidence-based practice guidelines. • Serves as a subject matter expert for wellness planning, grant proposals and other care coordination activities that target programs for persons with diabetes. • Supervises certified diabetes care and education specialists. • Provides direct patient care including hands-on education on continuous glucose monitors, insulin pumps, meters, blood glucose monitors, and medications. • Maintains credentials and expertise in education and self-management trends, technological devices and medications, including treatment adherence. • Prepares and submits required federal and state administrative reports and plans and responds to audit requests, including the American Diabetes Association and Centers for Medicare and Medicaid Services. • Ensures compliance with requirements of the ADA annual recognition program and Medicare for documentation and billing. • Tracks project progress and manages expectations through successful project completion, and provides contingency plans to ensure project objectives are met. • Interprets reports and dashboards and utilize the electronic health record to analyze data for quality improvement. • Manages Human Resources activities of department in regards to: recruiting and selection, hiring and termination, training, professional development, mentoring, counseling, performance evaluations, and salary planning. • Performs other duties as assigned.

Certification/Skills:

• Excellent written and verbal skills. • Registered Nurse (RN) State Licensure or Nurse Licensure Compact (NLC) by Texas Board of Nursing (BON) required or • Registered Pharmacist (RPh) by Texas State Board of Pharmacy (TSBP) required or • Registered Dietitian (RD) by Commission on Dietetic Registration (CDR) required • Licensed Dietitian by Texas Department of Licensing and Regulation (TDLR) required • Certified Diabetes Care and Education Specialist (CDCES) by Certification Board for Diabetes Care and Education (CBDCE) required • Board certification in a medical specialty Advanced Diabetes Management preferred Minimum Education: Graduate of an education program approved by the credentialing body for the required credential(s) requiredMinimum Experience: 5 years of related experience in diabetes education or chronic disease management required 1 year of supervisory/management experience preferredPhysical Requirements: Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.Security Sensitive: This position is a security-sensitive position pursuant to Texas Education Code §51.215 and Texas Government Code §411.094. To the extent that a position requires the holder to research, work on, or have access to critical infrastructure as defined in Texas Business and Commerce Code §117.001(2), the ability to maintain the security or integrity of the infrastructure is a minimum qualification to be hired for and to continue to be employed in that position.
