This is a rare and exciting opportunity to join the Functional Team within Older Peoples Community Services as a Clinical Team Lead.
Main duties of the job
The Clinical Team Lead will be responsible for the development and implementation of the day to day Clinical leadership of the Team, offering clinical leadership, ensuring the maintenance and promotion of high standards of person centred care for service users as well as assisting with the development of staff within the team. They will be expected to maintain, monitor and facilitate the delivery of a high quality Mental Health Services for older people.
The Clinical Team Lead will provide strong professional/managerial and clinical leadership to the Multidisciplinary Team and support with developing excellence in practice, ensuring that robust practice and personal development initiatives are available.
They will ensure national standards, and local guidelines, policies and care pathways relating to care and treatment are effectively delivered within Acute Mental Health Services and Memory Services for the promotion and delivery of quality services.

Job description Job responsibilities
The post holder will be responsible for the development and implementation of the day to day Clinical leadership of the Team.
They will offer clinical leadership, ensuring the maintenance and promotion of high standards of person centred care for service users as well as assisting with the development of staff within the team.
They will be expected to maintain, monitor and facilitate the delivery of a high quality Mental Health Service for Older people in the most efficient and effective manner within existing constraints.
To provide strong professional/managerial and clinical leadership to the Multi-disciplinary Team. To provide strong leadership in respect of developing excellence in practice, ensuring that robust practice and personal development initiatives are available. To ensure national standards, and local guidelines, policies and care pathways relating to care and treatment are effectively delivered within Acute Mental Health Services and Memory Services for the promotion and delivery of quality services. To promote and ensure service users and carers are fully involved in the services they receive. To support good quality care, by ensuring the highest standard of environment is provided, in line with National guidance and local service specification. To support and provide co-ordinated and integrated care, by ensuring effective working relationships exist between Acute Mental Health and Memory Services and the range of Community Services. To take the lead role in the promotion and implementation of person centred care. To ensure the care offered is at the level each one of us would want our own family to receive.
MAIN DUTIES / RESPONSIBILITIES:
CLINICAL:
To work across North East Lincolnshire providing mental health advice and leadership based on an agreed operational policy and evidence based practice.
To assist in developing and evaluation of models of care to enable effective communication and to facilitate best practice taking into account available resources.
Participate in multi-disciplinary team meetings, meetings associated with the Mental Capacity Act/Mental Health Act/Continuing Healthcare and team meetings.
To monitor, evaluate and ensure maintenance of the highest achievable standards of care.
Coordinate transfer or referral of service users to other teams or services where required. Prioritise, plan and coordinate own workload and activities in line with the changing needs of the team and service. This includes planning and monitoring client medication needs, making appointments for clients, conducting assessments, managing emergencies and enquiries, responding to crisis work within individual caseload, ensuring patient records are up to date and supporting colleagues on a daily basis. Have the ability to work independently and to be able to resolve complex professional problems. Crisis response to other staffs service users in the event of key workers absence, and delegation from manager. To participate in the implementation of an effective clinical supervision structure across the service
Assess skill mix and identify areas of development within the Team.
Participate in the continued development of a comprehensive Risk Assessment and Management strategy for the service.
Provide professional support to staff by devising and implementing a framework for critical incident analysis and de-brief.
To provide and set up appropriate clinical training for staff across the Team.
To take managerial responsibility for Older Peoples Services in the absence of the Senior Operational Manager.
To be a positive team member, act as a role model for all staff grades and to question concerns in a direct and responsible manner.
To ensure that all staff participate in a programme of PDR in line with NAViGO policy. To ensure that the actions and interventions of all staff are in accordance with relevant professional codes of conduct, the Mental Health Act (1983), the Mental Health Act Code of Practice and the Mental Capacity Act (2005) To work with and support the Senior Operational Manager with sound financial approaches to finances and budgets. To ensure that appropriate systems are in place in support of all aspects of day to day administration and forward planning of human resources in an effective way. To assist in the development of, and to ensure the implementation and monitoring of local policies on safety. To support providing a safe environment for service users, staff and visitors through effective Health and Safety at Work awareness and provision of good standard relating to the ward environment. Liaise with other members of the MDT and act as a link between the acute areas and CMHMS. Participate in meetings as appropriate and act as chair for relevant meetings. Facilitate the implementation of the new model of care for Older Peoples Mental Health Services and act as liaison between staff and senior management regarding the change process.
LEADERSHIP:
To provide ongoing support in line with Well Led expectations from the CQC. To provide on site accessible leadership and support for members of the Functional Team and also the wider Community and Memory Service team as required. To ensure a good working relationship is maintained with each team within the Older Peoples Acute and Community services to ensure resources and learning are shared as routine practice. To ensure that interventions meet local and national standards, and are based on an individualised approach to care, aimed at maximising an individuals potential and are based on effective engagement principles. To actively contribute, and lead, on the provision of positive and effective multi-disciplinary approaches to the delivery of care and treatment within the team. To actively contribute to the establishment and maintenance of practical and meaningful clinical governance initiatives within the team. To attend and contribute to the regular Quality Learning Network meetings. To be able to accurately interpret data and apply this to service development plans. To work constructively with the senior professional colleagues in the delivery of team approaches to service improvement, practice development, audit and the delivery of expected service standards. To ensure monthly supervision is delivered to team staff via a fairly distributed supervision tree. To ensure support is provided to staff in regards to workforce concerns- to lead on workforce investigations and reviews. To escalate concerns to the SOM as required. To manage the capability and competency of staff members effectively using the policies and procedures in place. Working closely with Workforce and the SOM to do so. To take an active role in the leadership of reviews of care- whether these be After Action Reviews, Summary Judgement Reviews (SJRs) or Patient Safety Incident Investigations (PSIIs). To provide clinical oversight to complex cases being managed by the team. To liaise closely with members of the Safeguarding Team. To take part in the On Call Managers rota to provide out of hours support to Navigo. To assist in the leadership approach to safe and effective Infection Control principles. To work with Senior Management to ensure staffing levels are safe and any variations are reported appropriately.
Person Specification
Knowledge & Understanding Essential
Ability to organise, prioritise and delegate. Up to date knowledge of policy and procedure as it relates to service area with the ability to contribute to development of local policies. Ability to demonstrate an understanding of the principles and values of clinical supervision. Ability to demonstrate competence in accurate record keeping. Commitment to anti discriminatory practice, with knowledge and understanding of cultural and equal opportunities issues. Commitment to continuous professional development.
Desirable
Previous experience in supervisory role. Undertaken recognised Leadership/Management course.
Qualifications Essential
Registered Nurse/Occupational therapist/Social Worker IT Skills 1 year experience of leadership at Band 6 2 years minimum relevant experience post qualification Portfolio of relevant post qualifying education Degree or equivalent
