Job summary
An exciting opportunity has arisen for an experienced and highly motivated registered healthcare professional to join our Temporary Staffing Department as a Bank Band 7 Single Point of Access Clinical Practitioner.
Based at Priestley Wharf, you will provide advanced clinical expertise and leadership within the Single Point of Access service. You will undertake clinical triage, assessment and care planning for patients with acute, undiagnosed and long-term conditions, helping to prevent unnecessary hospital admissions and support timely discharge from hospital.
This is a flexible bank position covering a seven-day service. You will be expected to work shifts according to service requirements, normally between 08:00 and 22:00. You will also rotate into clinical teams every three to four months to maintain your knowledge of community services and your clinical practice.
Main duties of the job
You will use advanced assessment, professional judgement and clinical decision-making skills to prioritise referrals, identify risks and determine the most appropriate pathway of care.
Your principal duties will include:
Providing safe and effective telephone triage and advanced clinical assessment through a range of consultation methods. Taking comprehensive patient histories and assessing people with acute, complex or undiagnosed healthcare needs. Initiating appropriate investigations, interpreting diagnostic results and evaluating care, with support from the Advanced Clinical Practitioner where required. Making direct referrals for medical assessment, diagnostic procedures and other services within agreed clinical pathways. Prescribing independently and undertaking medication reviews as part of patient assessment and monitoring. Recognising and responding to complex, urgent and emergency situations, including initiating effective emergency care. Supporting patients, carers and professionals to make informed decisions about care. Signposting and referring patients to appropriate health and care services to reduce avoidable attendances and hospital admissions. Managing acute caseloads and coordinating discharge or transfer to appropriate internal or external services. Safeguarding children and adults at risk by identifying, reporting and acting immediately upon concerns. Maintaining accurate, complete and contemporaneous clinical records.
