Job summary
An exciting opportunity has arisen for a motivated and experienced Band 6 Nurse to join the Urgent Care Bureau (UCB) Single Point of Access (SPA) team. This role is central to delivering high-quality, patient-centred care within community settings, helping to prevent unnecessary hospital admissions and supporting patients to remain safely in their own homes.
As an autonomous practitioner, you will provide comprehensive telephone triage and clinical assessment, making timely and effective clinical decisions for patients with a wide range of complex physical and mental health needs. This is a rewarding role within a dynamic, multidisciplinary service operating across a seven-day week and providing 24-hour support.
Main duties of the job
The successful candidate will:
Provide expert telephone triage and clinical assessment for patients referred into the Single Point of Access service. Make autonomous clinical decisions, ensuring patients receive the most appropriate care pathway and support. Identify signs of deterioration, clinical risks, and safeguarding concerns, escalating appropriately when required. Work collaboratively with GPs, acute hospitals, community services, social care, and voluntary sector partners. Coordinate referrals and facilitate access to community services to support patient outcomes. Deliver evidence-based care and contribute to service improvement and quality initiatives. Support the development of colleagues, students, and newly appointed staff through supervision, mentoring, and education. Participate in audits, quality improvement projects, and clinical governance activities. Contribute to the delivery of a responsive seven-day service through flexible working patterns, including weekends and bank holidays.

As a Band 6 UCB/SPA Nurse, you will provide specialist clinical assessment and triage via telephone, ensuring patients receive timely and appropriate care within community settings. You will assess complex referrals, identify risks and "red flags", and determine the most suitable care pathway, including escalation to emergency services where required.
The role requires excellent clinical judgement, communication skills, and the ability to work independently while remaining closely connected to a wider multidisciplinary team. You will manage referrals from a variety of healthcare professionals, support patients with complex physical and mental health needs, and provide reassurance and expert advice to patients, carers, and referrers.
You will contribute to service development through clinical governance, audit, quality improvement initiatives, and the implementation of evidence-based practice. In addition, you will support the education and development of colleagues and students, act as a role model within the team, and assist with the day-to-day operation of the service when required.
The post operates on a flexible rota covering seven days per week and contributing to a 24-hour service, requiring a commitment to working evenings, weekends, and bank holidays as needed. The successful candidate will be passionate about delivering excellent patient care, promoting independence, and working collaboratively with partners across health and social care to achieve the best outcomes for patients.
