About this role

Job summary

The Occupational Therapist in the Long Term OT Team, will support adults with complex, long-term conditions and disabilities to live safely and independently in their own homes.

This role holder will manage a specialist caseload, completing person-centred, strengths-based assessments and delivering practical interventions tailored to individual goals.

This includes prescribing and monitoring equipment, promoting assistive technology, supporting safe transfers of care and maintaining accurate clinical records. Working collaboratively with service users, carers and partners across health, housing, social care and the voluntary sector, you will contribute to coordinated care, service improvement and high-quality professional practice, while providing guidance and supervision to Occupational Therapy Assistants where appropriate.

Conduct holistic, person-centred assessments of service users and carers in their homes.Manage a specialist caseload involving adults with complex, long-term conditions and disabilities.Develop strengths-based support plans and set SMART therapeutic goals.

Job description Job responsibilities

Deliver interventions that promote safety, independence and quality of life. Prescribe, provide and monitor disability equipment and adaptations safely. Promote assistive technology and digital tools, such as AskSARA, to reduce reliance on formal care. Apply manual handling legislation, training and best practice to all relevant interventions. Work collaboratively with carers and professionals across health, housing, social care and voluntary organisations. Contribute to multidisciplinary assessments, treatment planning, care pathways and safe transfers or discharges. Maintain accurate, timely and up-to-date clinical records. Participate in supervision, appraisals, peer review and continuing professional development. Provide supervision and guidance to Occupational Therapy Assistants where appropriate. Contribute to service development, quality improvement and cost-effective care.

Skills and abilities Essential

Experience of diffusing conflict and complaints. Problem solving/decision making skills based on client needs and goals. Excellent assessment and recording skills Demonstrates positive attitudes and behaviours, taking a strength-based approach to assessments. Willingness to take responsibility and use initiative, according to level of experience and delegations Ability to communicate effectively with people with complex difficulties and in difficult situations. To be proficient in the use of ICT systems and packages Self-motivated with the ability to work independently and influence others. Excellent time management skills, including the ability to work to deadlines. Ability to work in a fast-paced environment and concentrate in conditions requiring emotional demands. Ability to work autonomously, prioritising and organising own work and caseload, including delegation. Physically capable of carrying, transporting and fitting equipment in peoples homes, within manual handling constraints and guidelines

Qualifications Essential

Degree/Diploma/MSc in Occupational Therapy. HCPC Registered. Commitment to ongoing professional development.

Masters level qualification in specialist area.

Knowledge and understanding Essential

The ability to converse at ease with customers and provide advice in accurate spoken English. Knowledge of relevant legislation, national guidelines and best-practice for a range of conditions. Comprehensive knowledge of appropriate and available Occupational Therapy assessments and approaches, including outcome measures and goal setting. General knowledge of safe installations and use of Assistive Technology, disability equipment and adaptations

Can demonstrate and evidence skills in working with all age groups and many pathologies and disabilities Experience of working with people of diverse cultural and religious communities Proven ability in implementing Clinical Governance. Experience of contributing to service/quality improvement. Experience of working within a community or acute therapy team. Experience of case-management in clients with complex long-term conditions and disabilities Experience of working with staff and managers at all levels across the community, engaging with health, housing, partner agencies, and voluntary sector.
