Job summary
The Reablement Occupational Therapist supports adults to regain and maintain independence in their own homes. The role involves assessing service users and carers needs, managing a specialist clinical caseload, setting outcome-focused goals, and recommending therapeutic interventions, equipment, adaptations and assistive technology. Working closely with health and social care colleagues, the postholder helps deliver safe, strengths-based and cost-effective support, promotes timely discharge from services, maintains accurate clinical records, and may supervise Occupational Therapy Assistants.
Main duties of the job
We are seeking a passionate and skilled Occupational Therapist to join our Reablement and Independence Service RIS. This community based role focuses on empowering individuals to regain or maintain independence in their own homes through strength-based assessments, rehabilitation, and recovery-focused interventions.
Working within a multi-disciplinary team MDT, you'll support residents with a range of needs including physical and mental health conditions, learning difficulties, and age-related challenges. You'll also collaborate with colleagues across the Local Access Point, Locality and Hospital Social work and Discharge to Assess teams to ensure seamless care transitions and effective discharge planning.

The postholder will identify the needs of the service users and carers within the context of their own homes and within available resource.To involve carers in this process and consider their needs as well as those of the service users to ensure best outcomes.To manage autonomously a clinical caseload of clients at a specialist level, assessing and treating clients and maintaining clinical records.Participate in clinical supervision and appraisals and supervise OT Assistants according to level of expertise.To ensure that all activities are informed by up-to-date manual handling advice and training.To set SMART goals to ensure therapeutic interventions are effective and to improve client flow through the service.Promote the use of Assistive Technology and AskSara to enhance independence and reduce reliance on formal care services.To work collaboratively with health and social care community and acute colleagues, including Local Access Point and Discharge to Assess teams.To work with other members of the MDT to ensure effective assessment, treatment, management of care pathways and transfers of care discharges.
Person Specification
Qualifications Essential
Degree/Diploma/M.Sc. in Occupational Therapy. HCPC Registered. Commitment to ongoing professional development.
Desirable
Masters level qualification in specialist area.
Skills and abilities Essential
Proven ability in implementing Clinical Governance. Problem solving/decision making skills based on client needs and goals. Excellent assessment and recording skills Demonstrates positive attitudes and behaviours, taking a strength-based approach to assessments. Willingness to take responsibility and use initiative, according to level of experience and delegations Ability to communicate effectively with people with complex difficulties and in difficult situations. To be proficient in the use of ICT systems and packages Self-motivated with the ability to work independently and influence others. Excellent time management skills, including the ability to work to deadlines. Ability to work in a fast-paced environment and concentrate in conditions requiring emotional demands. Ability to work autonomously, prioritising and organising own work and caseload, including delegation. Physically capable of carrying, transporting and fitting equipment in peoples homes, within manual handling constraints and guidelines
Experience Essential
Recent experience demonstrating ability to assess and evaluate care in rehabilitation and long-term condition management. Experience of working with people of diverse cultural and religious communities Experience of contributing to service/quality improvement. Experience of working within a community or acute therapy team. Experience of diffusing conflict and complaints. Moving and Handling case work experience.
Knowledge and understanding Essential
The ability to converse at ease with customers and provide advice in accurate spoken English. Knowledge of relevant legislation, national guidelines and best-practice for a range of conditions. Comprehensive knowledge of appropriate and available Occupational Therapy assessments and approaches, including outcome measures and goal setting. Experience of case-management in clients with long-term conditions. General knowledge of safe installations and use of Assistive Technology, disability equipment and adaptations
