We are seeking an enthusiastic and highly organised Clinical Flow Coordinator to join our Care Coordination Centre (CCC) based at St Margaret's Hospital, Epping. This is an exciting opportunity to play a pivotal role in supporting system-wide patient flow across West Essex, working closely with multidisciplinary teams, community services, acute providers, local authorities and wider health and care partners. The role focuses on coordinating safe transfers of care, supporting discharge processes, monitoring patient pathways and helping to manage operational pressures across the health and care system.

This is a fixed-term contract until 31 March 2027 with the potential for extension, working 37.5 hours per week, Monday to Friday.

There are 2 positions available, 1 full time and 1 part time (22.5 hours).

Main duties of the job

As a Clinical Flow Coordinator, you will play a key role in supporting the effective operation of the Care Coordination Centre (CCC) and Transfer of Care Hub, helping to improve patient flow across the West Essex health and care system. Working closely with clinicians, operational managers and partner organisations, you will support admission avoidance, discharge planning and the safe transfer of care for patients.

You will maintain patient flow trackers and operational systems, ensuring accurate and timely data to provide oversight of demand, capacity and patient outcomes. The role involves coordinating multidisciplinary activities, monitoring patient progression through care pathways, escalating delays or risks, and ensuring agreed actions are completed.

You will also support referral screening, bed management processes, service improvement initiatives and the production of reports and information to support operational decision making. Working collaboratively with colleagues across community, acute, social care and voluntary sector services, you will help ensure patients receive the right care, in the right place, at the right time.

The Clinical Flow Coordinator plays an integral role within the Care Coordination Centre (CCC), supporting the safe and effective movement of patients through health and care services across West Essex. The post holder will work alongside clinicians, operational managers and system partners to coordinate patient flow, support admission avoidance initiatives, facilitate timely discharge, and ensure patients receive the right care at the right time.

The role is responsible for maintaining oversight of patient activity, demand, capacity and outcomes through the accurate management of patient flow trackers and operational systems. The successful candidate will support referral screening, bed management processes, multidisciplinary team meetings and transfer of care activities across community, acute and social care services.

Working within a fast-paced operational environment, the post holder will:

Coordinate patient flow activity and support safe transfers of care across the health and care system. Maintain accurate patient tracking systems, ensuring data quality and timely updates. Monitor operational pressures, capacity constraints and risks, escalating concerns appropriately. Support admission avoidance initiatives and discharge planning processes. Produce reports, data analysis, briefing papers and presentations to inform operational decision-making. Organise and support multidisciplinary team meetings, maintaining oversight of actions and outcomes. Work collaboratively with internal and external stakeholders, including community teams, hospital services, local authorities, the Integrated Care Board and voluntary sector partners. Contribute to service improvement initiatives, helping to identify opportunities to enhance patient flow, system performance and patient outcomes. Support clinicians and managers with referral management, pathway coordination and operational assurance activities. Person Specification

Knowledge and Experience Essential

Experience of working in a support role to a multidisciplinary team Ability to produce briefing packs and materials, draft reports, data, letters and presentations Experience of working in a similar role in a health or care context.

Education/Qualifications Essential

Educated to a degree level in relevant discipline or equivalent level of demonstrable experience of working at a similar level

Communication Skills Essential

Excellent interpersonal and oral/ written communication skills with the ability to communicate complex/sensitive information. Ability to deliver against agreed objectives achieving quality outcomes. Excellent interpersonal skills with the ability to work with colleagues at all levels. Minute taking skills.

Personal and People Development Essential

Ability to manage workload effectively in light of competing priorities and tight deadlines. IT skills including Word, Excel, PowerPoint etc.
