Job summary

An exciting and challenging opportunity has arisen for an experienced physiotherapist to join our multidisciplinary specialist community respiratory care team. The office is currently based in Walton Community hospital, Walton on Thames

This team covers the North West Surrey area to support the management of patients with chronic respiratory disease.

You will need experience of managing patients with respiratory disease ideally with an understanding of pulmonary rehabilitation, oxygen and non-invasive ventilation.

The successful candidate must have good communication and organizational skills with a flexible approach to meet the ever-changing needs of the service.

Experience of working autonomously and within a multidisciplinary team is essential.

Physiotherapists are an integral part of the team who provide assessments, treatment and self-management education in community clinics, home visits, provision of Pulmonary Rehabilitation and Ambulatory Oxygen clinics

The physiotherapists in the team also support as needed with admission avoidance, post discharge support and Non invasive ventilation services

The team strives to ensure evidence based practice and audits their service regularly.

Main duties of the job

The Respiratory Care Team is a small multidisciplinary team based at Walton Community Hospital. As the Band 6 Physiotherapist, your main duties will be to assess, treat and support patients with respiratory conditions such as COPD, Asthma, Bronchiectasis and Pulmonary Fibrosis in a variety of settings. This includes outpatient clinics, home visits, ambulatory oxygen reviews, NIV service, admission avoidance and post discharge support via telephone and running Pulmonary Rehabilitation exercise and education sessions.

You will undertake comprehensive respiratory assessments, including spirometry and interpretation of results, develop personalised treatment plans, and provide expert advice to patients, carers and healthcare professionals. A key aspect of the role is empowering patients to self-manage their condition through education, health promotion and behaviour change support.

Strong communication skills are essential, as you will be required to explain complex and sensitive information to patients and families, helping them make informed decisions about their care and rehabilitation. You will maintain accurate clinical documentation, always uphold patient dignity and confidentiality, and contribute to the continuous improvement of services by applying current national respiratory guidelines and best practice.

Job description Job responsibilities

Purpose:

1. To undertake comprehensive physiotherapeutic assessments and to plan and execute treatment of patients with chronic respiratory problems in outpatient, clinic and community settings. These patients may have complex needs, both physical and emotional, including co-morbidities and may be at any stage of the disease process, including end stage.

2.The Band 6 Respiratory Physiotherapist supports the Pulmonary Rehabilitation Lead within the locality area to plan, co-ordinate, deliver and evaluate a high quality, evidence based Pulmonary Rehabilitation and respiratory physiotherapy service for respiratory patients in North West Surrey.

3. To have responsibility for providing pulmonary rehabilitation sessions, both face to face and virtually for patients with respiratory conditions across NW Surrey.

4. To contribute to the overall planning, strategic development, and evaluation of the Pulmonary Rehabilitation Service in conjunction with other clinical staff.

5. To take an active role in the supervision and teaching of physiotherapists, nurses and other members of the multidisciplinary team regarding respiratory problems across the organisation.

6. To be responsible as an autonomous practitioner to undertake a significant specialist clinical workload.

7. To be an active member of the respiratory care team, including participation in supporting unplanned admission avoidance where appropriate, providing post discharge support, community clinics, home visits and Ambulatory Oxygen clinics.

8.To contribute, once able, to the provision of an effective high quality Home Non-invasive Ventilation service

9.To participate actively in evaluating the quality and effectiveness of care delivered and make recommendations for changes.

10.To undertake evidence based projects, including recommendations for change in practice and to lead where appropriate in departmental evaluation/research.

Primary duties and areas of responsibility:

To deliver autonomous evidence-based patient care, achievable through direct patient contact To communicate effectively with all disciplines involved in a patients care both those within the organisation and those outside to ensure effective multidisciplinary working To be able to communicate complex and sensitive information to patients, carers and other staff including imparting unwelcome news relating to their rehabilitation potential To use persuasive and influencing skills to encourage patients to self-manage their conditions To be able to carry out spirometry and interpret spirometry results To work in conjunction with the multidisciplinary team across both primary and secondary care To be responsible for assessing patients respiratory needs referring them to other health care professionals as appropriate Provide autonomous advice and treatment recommendations with regards to patients specific respiratory and education needs in order to improve health outcomes. Promote health through empowering patients, offering appropriate health education, information and advice. To keep accurate, electronic and written documentation, care plans and reports

To support the management of COPD exacerbations and other chronic respiratory conditions and support the discharge process from secondary care. To involve, support and educate patients and, as appropriate, their family and/or carers To treat all patients as individuals, respecting their privacy and dignity at all times To promote health and wellbeing to the patient and their family/carers To have an awareness of current national standards and guidelines relevant to Respiratory Disease and improve practice and health outcomes where appropriate.
