About this role

Job summary

Are you an enthusiastic, registered Mental Health Nurse /Community Psychiatric Nurse looking to work across teams within a friendly service? The Windsor, Ascot and Maidenhead (WAM) Older Peoples Mental Health Service is looking for a motivated Nurse to join our friendly and innovative team.

You will work in our Older Adults Home Treatment Team providing mental health care and treatment using appropriate pathways including the Dementia Living well Strategy in line with NICE guidelines.

The role is full-time. The service operates 7 days per week, so flexibility regarding unsocial hours/ shift patterns will be required.

You may be required to take a lead in an area of special interest, such as working with carers or Younger People with Dementia. You will be responsible for ensuring clinical governance guidelines are followed and to be involved in delivering training and educational programmes as well as participating in audit and research.

The role is based at Nicholson House, Maidenhead and your time in this role will be spent visiting clients in their homes in the local community so you will need a car to get to appointments. You will be required to come to the office occasionally for team meetings, training or for other service-related reasons as necessary, although currently we conduct most team meetings virtually this may change in future.

The successful postholder would need a valid UK driver's license and their own car to successfully carry out home visits.

Main duties of the job

Participating as a member of a multi-disciplinary team managing a caseload of patients Undertake assessments of patients, involving their carers and family members in the patient's care and care plans. To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team. Supervising Support Workers and/or students. Providing advice to other members of the service and agencies on the needs of service users.

The must haves for this role:

Registered Mental Health Nurse or equivalent. Good understanding of the relevant legislation surrounding the Mental Capacity Act / Mental Health Act. Ability to drive, with valid UK drivers license and have a car for use at work. For further information about the role, please see attached job description and person specification.

Proficient use of digital tools and applications including Word, Outlook and patient management systems.

Additional Requirements Essential

Ability to travel between multiple Trust sites and non-Trust locations, including patient homes, some of which may not be easily accessible via public transport.
