About this role

Job summary

Are you passionate about helping people regain their independence and achieve their rehabilitation goals? We are looking for an enthusiastic and motivated Rehabilitation Assistant to join our friendly and supportive Community Therapy Team covering Petersfield and Bordon.

Working alongside Occupational Therapists, Physiotherapists and other healthcare professionals, you will play a key role in supporting patients with a wide range of rehabilitation needs within their own homes and community settings. You will deliver delegated rehabilitation programmes, support mobility and functional activities, monitor patient progress and contribute to achieving positive outcomes for patients.

You will need excellent communication and organisational skills together with a compassionate, person-centred approach to care. Previous experience working within a health, social care or rehabilitation setting is desirable, although full support, supervision and development opportunities will be provided.

A full UK driving licence and access to a vehicle for work purposes is essential.

Main duties of the job

*Work under supervision from the Therapy Team Leader, Senior Therapists, and Therapists .

*Implement Therapy to a designated caseload:

E.g. therapeutic handlingParticipation in treatment activity*Ensuring patients and carers are an integral part of their treatment planning and goal setting.

*To be responsible for issuing standard aids.

* To ensure that up to date records, reports and activity data are maintained in accordance with professional and Trust standards.

*To provide intervention in hospital/healthcare setting and where appropriate, in client's home to meet client needs.

*to ensure smooth running of the team such as patient registration, liaison reporting and recording data etc.

*adhere to manual handling guidelines.

*support the work of the practitioners in the team.

*prepare patients for and assist with clinical/therapeutic activities or treatments.

*Demonstrates skills and knowledge to contribute too and lead effectively the Safeguarding Process.

*Awareness and application of a range of local / national policy and procedural frameworks when undertaking Safeguarding activity.

*Ensure service users are supported appropriately to understand Safeguarding issues to maximise their decision making.

*Understand when to use emergency systems to Safeguard adults and children / know when to call for ambulance and police intervention / contact out of hours service / decide when emergency plans may be required and use legislation where immediate action may be required.

Understanding of Health and Safety and relevant NHS policies, Trusts and service standards, policies, protocols and guidelines. Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being. Knowledge of National agendas and the effect on practice e.g. NSF's including Chronic Disease Management, and the Public Health Agenda
