We are looking for a Band 5 Discharge Co-ordinator to join our friendly Inner London Inpatient Rehabilitation Service, working across Athlone and Alexandra Rehabilitation Units.
As thesole Discharge Co-ordinator across both units, you will have the opportunity to take ownership of discharge coordination while working closely with a supportive multidisciplinary team. This includes Occupational Therapists, Physiotherapists, Rehabilitation Nurses, GPs, Nursing and Rehabilitation Assistants, Speech and Language Therapists, Dietitians and Podiatrists. You will be closely supported by the Clinical Therapy lead and Matron. You will need strong communication and interpersonal skills and a commitment to providing a positive patient experience.

Main duties of the job
As the sole Discharge Co-ordinator across Athlone and Alexandra Rehabilitation Units, you will play a key role in supporting safe, timely and well-coordinated discharges. Working collaboratively with the multidisciplinary team and external partners, you will help patients make a smooth transition from the units.
You will:
Coordinate the patient journey from admission through to discharge. Work with the multidisciplinary team to support positive patient outcomes and communicate assessment outcomes and discharge plans with those involved in their care. Support the Clinical Therapy Lead, Matron and Team Leads with improvements to patient experience and discharge processes. Liaise with external agencies, discharge hubs and social services to support effective patient flow. Work with transport and equipment providers to support timely and smooth discharges. Produce reports and collect data to demonstrate safe and effective discharges, escalating concerns where appropriate.

Person Specification
Qualifications Essential
NVQ level 3 qualification in health and social care or equivalent, or 2 or more A levels Evidence of CPD.
Desirable
Training in discharge planning or case management. Use of hospital IT systems e.g. System One BSc, BA honours degree or equivalent Professional registration as OT, PT, nurse or social worker
Experience Essential
Ability to demonstrate a clear understanding of the role of a discharge co-ordinator. Experience working with different healthcare professionals and understanding of the roles of the multi-disciplinary team (MDT) within a Rehab setting. Ability to establish a good rapport and professional working relationships, in a clinical setting, with patients, family and friends, therapy, nursing and all members of the MDT who are involved in patient care. Experience working autonomously and as part of a team including the ability to work under supervision and be aware of limitations seeking help as appropriate. Experience of working in a team. Experience of working with adults in health, social or educational setting either in paid or voluntary capacity.
Desirable
Experience working in the NHS Experience working in a Discharge Team or in a Discharge co-ordinator, progress chaser, pathway co-ordinator role Knowledge of the Continuing Healthcare process and documentation Experience in working with patients with physical disability or challenging behaviour within a rehabilitation setting. Experience liaising with external agencies to support the service users needs such a social services, charitable organisations etc.
Skills Essential
Basic IT skills - Microsoft Word, Power point, Excel Good verbal and written communication skills, including the ability to keep legible and accurate patient records.
