Job summary

Community services in Hammersmith & Fulham are evolving through the development of Integrated Neighbourhood Teams (INTs), creating an exciting opportunity for a Community Matron to help shape how care is delivered locally.

You will join a forward-thinking, nurse-led team where your clinical expertise and leadership can make a real difference to people with complex health needs, supporting them to receive coordinated care and remain in their preferred place of care wherever possible.

Working across organisational boundaries, you will help strengthen collaboration between community services, primary and secondary care, social care and voluntary sector partners.

As part of CLCH, one of London's largest community NHS trusts, you will be supported to develop integrated ways of working, contribute to service improvement and champion high-quality, evidence-based care.

We welcome applications from nurses with the required advanced clinical and prescribing skills who are motivated by collaborative working and improving outcomes for people and communities.

Main duties of the job

As Community Matron, you will use advanced assessment, clinical decision-making and prescribing skills to coordinate care for people with complex needs and long-term conditions. Working with patients, carers and multidisciplinary partners, you will support personalised care, help prevent avoidable hospital admissions and facilitate timely discharge.

Key responsibilities include:

Provide expert assessment, evidence-based treatment and complex case management. Act as named Community Matron for an Integrated Neighbourhood Team (INT), supporting care coordination across the network. Work collaboratively with community and district nursing, primary and secondary care, mental health services, adult social care, voluntary sector partners and London Ambulance Service. Contribute to ICAP and other multidisciplinary meetings and high-intensity user forums. Support people with conditions including COPD, diabetes, Parkinson's and coronary heart disease. Provide education that supports patients and carers with self-management and builds capability across health and social care teams. Provide clinical leadership and promote safe, high-quality practice. Monitor care quality, identify opportunities for improvement and contribute to service development.

Person Specification

Education/Qualification Essential

Registered Nurse Evidence of continued professional development Relevant post-registration qualification (e.g. advanced clinical assessment skills, district nursing specialist qualification, frailty)

Experience Essential

Extensive experience of assessing and providing care to people with complex long-term conditions Experience of effectively working in collaboration, negotiating across organisational boundaries Community experience Experience of MDT working

Skills & Knowledge Essential

Evidence of a high level of clinical skills and expertise Understanding of frailty Ability to undertake holistic assessments Ability to undertake holistic assessments Ability to effectively communicate difficult information both verbally and in writing Effective skills in delegating, negotiating, facilitating, and teaching
