About this role

Job summary

Are you an experienced Support Time and Recovery Worker passionate about improving the physical health of people experiencing mental health difficulties?

We are looking for a motivated, enthusiastic individual to join our Early Intervention in Psychosis (EIP) service team as a Physical Health Specialist, a unique development opportunity for those with significant mental health experience and physical health skills, particularly in phlebotomy, health promotion and supporting lifestyle change.

You will work closely with service users and the multidisciplinary team to deliver physical health monitoring, health interventions and personalised wellbeing programmes, helping to reduce health inequalities and improve outcomes for people with severe mental illness.

We are particularly interested in applicants who:

Have substantial experience working in mental health services.

Are confident engaging with service users with complex needs.

Have phlebotomy skills or are willing to undertake training.

Have an interest in physical health, fitness, wellbeing, and health promotion.

Are committed to professional development.

This is a development post and the successful applicant will be supported through Trust funded physical health training and qualifications and competency development to progress to Band 4 Physical Health Specialist.

Main duties of the job

Understand the processes of physical health monitoring in EIP teams.

To implement health coaching and physical activity interventions and lifestyle modification to support service users to make life changes to promote better health outcomes.

To undertake physical health checks using processes that follow SHFT and national guidelines.

To contribute to the collection and analysis of information relating to physical health monitoring and compliance with policy to support service evaluation and improvement.

To facilitate and/or provide health promotion and education to service users and staff. To support colleagues within the EIP team to complete physical health checks, particularly with those service users who are difficult to engage or are in isolated locations with limited public transport.

To carry out any duties as specified to ensure physical health is monitored in line with best practice and to reduce the mortality gap To ensure that physical health monitoring outcomes are communicated with primary care in a timely manner.

To use ICE or relevant systems to request testing to ensure timely monitoring and results being communicated effectively. To collaboratively create and deliver personally tailored physical health action plans to service users in the team.

Liaise with a wide range of internal and external stakeholder to develop an understanding of available physical health interventions.

Person Specification

Qualifications Essential

Good literacy and numeracy skills. Previous experience working in a care setting. Phlebotomy trained, or willing and able to complete phlebotomy training. Ability to fulfil the travel requirements of the post. Willingness and ability to complete a recognised Personal Training and Coaching qualification. Successful completion will support progression from Band 3 to Band 4, subject to meeting all other requirements.

Desirable

Knowledge of Lester tool/cardiometabolic monitoring Registered with CIMSPA- Chartered Institute for the management of sport and physical activity

Experience Essential

Experience of working with mental health service users Experience of supporting physical health interventions Ability to undertake physical health observations Ability to engage hard to reach service users Knowledge of severe mental illness and physical health inequalities Able to facilitate group work Up to date on national best practice and guidelines applicable to service pathway Demonstrable skills in using clinical record systems and performance monitoring systems, for example Insights, and RiO/SystmOne. Demonstrates excellent problem-solving skills, with the ability to respond to unexpected challenges and make sound judgments about when to act independently or escalate. Skilled in interrogating, analysing, and validating data from various computer-based record systems to ensure accurate reporting and performance monitoring. Consistently considers how their work can support clinicians in delivering better outcomes for service users. Committed to high standards, promoting quality in all aspects of work Able to produce comprehensive reports, data summaries, and formal correspondence with clarity and professionalism.

Experience of working with service users presenting with psychosis Experience of behaviour change interventions Knowledge and awareness of organisations which provide physical health support and interventions
