About this role

Job summary

We have an exciting opportunity for an experienced nurse with an interest in Homeless Health to join our specialist Homeless Pathway Team here at LNWH. for a 7 month Secondment. You will be part of an established and supportive team, working with people experiencing homelessness within the hospital to improve their health outcomes, patient experience, and to work alongside our partners to support the flow of discharge into the community.

Main duties of the job

The successful candidate will use expert clinical skills in assessing and creating treatment plans. This includes referring and signposting to the relevant clinical and non-clinical areas including liaising with our health, social, housing, and specialist (such as substance misuse) partners, amongst others in the third sector.

They will manage a caseload and use their skills in active listening to engage and advocate for the patients.

This position requires being a role model; teaching and advising other staff and partners and working collaboratively with patients who may have highly distressing personalcircumstances.This includes leading the team and having oversight of the caseload, providing expert advise and offering regular supervision and training to other staff members, including students.

Other responsibilities include timely and accurate documentation, data entry and collection into various clinical systems.

The post holder will be expected to lead on the delivery of specific service targets e.g. GP registration for patients, attend multi-agency meetings as required and lead as necessary.There will be opportunity to assist in producing ad hoc reports and in project work.
