About this role

Job summary This role supports the delivery of high-quality patient care by providing support within the multi-disciplinary anticoagulation team and delivering point of care community INR clinics. The post holder will work closely with colleagues within the team and across the hospital and community to ensure we provide a safe and efficient anticoagulation service for our patients.

Main duties of the job The postholder will be expected to deliver POCT INR clinics in the community and perform INR tests in patient's home if required. Therefore daily use of own car is required. They will need to be reliable, punctual, responsible and well organised and used to working in a healthcare setting. The role requires you to work alongside registered HCP's in the Anticoagulation office, telephoning patients, booking appointments and supporting safe and effective care. The postholder will also support the processing of coagulation samples in the Laboratory when required. Attention to detail and careful documentation is essential.

Detailed job description and main responsibilities

To perform point of care tests (POCT) for INR for those patients on certain anticoagulants within the patient’s home or community clinic. (own car required) Be responsible for the correct and safe use of POCT INR meters. Be responsible for ensuring you have all equipment, consumables and patient lists for your clinics. Seek information from the patient regarding their anticoagulation whilst performing their POCT INR. Use laptops and upload results whilst in clinics. Liaise closely with the office to improve the anticoagulation control of all our patients To accurately record results of INR tests and patient information. To take venous blood samples from patients in their home or in community clinics, according to protocols, practices and standards required by pathology, when necessary. To follow correct procedure for the positive identification of patients. Ensure infection control procedures are carried out according to policy and correct PPE is worn at all times. Ensure Trust uniform policy is strictly adhered to in clinics. Be able to work alone at POCT INR clinics if required. Know the department and trust procedures for supporting patients who become unwell and know how to obtain assistance. Enter patient demographics and tests onto laboratory computer system. Attention to detail is paramount. Prepare samples for laboratory including labelling with barcode where appropriate, and ensure timely flow into the laboratory. Label samples with patient demographics in line with procedures. Sort and centrifuge samples for INR testing and load onto analyser. Update and maintain patient records on DAWN database Train and mentor other staff members in all aspects of Anticoagulation domiciliary duties Complete and maintain your competency assessments. Attend in house-training and updates for anticoagulation. Attend departmental meetings. Assist in audits to maintain a quality service. Ensuring cost effective use of resources and awareness of stock levels to ensure the smooth running of the service. Observe and maintain clean and tidy work areas. Rotate as requested through the various POCT clinics and work areas according to the needs of the service. Duties include answering the telephone in a professional and courteous manner. Documentation of phone calls on the DAWN system. Other duties as directed by senior anticoagulation staff. To work flexibly according to the demands of the service and be flexible in covering clinics at short notice, when necessary Monitor stock of reagents and consumables and assist in ordering stock, ensuring there is always an adequate supply.

Person Specification

Qualifications Essential

Maths and English Level 2 NVQ Level 2/3 or equivalent

Experience Essential

Able to communicate effectively with patients and colleagues verbally and in writing skills Previous experience in healthcare/ working as a care provider

Desirable

Phlebotomy experience
