# Physical Therapist – Home Health | $100–$115 Per Visit **BrightStar Care of Texas** is looking for a compassionate, skilled Physical Therapist to join our dedicated home health team. • Conduct comprehensive physical therapy evaluations and develop individualized treatment plans • Deliver skilled PT services to home health patients across a variety of diagnoses and conditions • Monitor patient progress and adjust care plans accordingly • Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated, patient-centered care • Maintain accurate, timely clinical documentation in compliance with state and federal regulations • Educate patients and family members on home exercise programs and safe mobility techniques --- ## Qualifications
• **Active Texas Physical Therapist (PT) license** — required • Home health experience preferred; new graduates with clinical experience are welcome to apply • Strong communication, time management, and organizational skills • Reliable transportation and valid driver's license • CPR certification preferred --- ## What We Offer
