You would be joining a very friendly, supportive multi-disciplinary team, to help with demands of same day appointments, as well as the ongoing requirements of general practice.
As an Advanced Practitioner you will exercise advanced clinical expertise, acting within your professional boundaries, providing advanced clinical skills and advice.
If you are passionate about improving patients health outcomes through the demonstration of the sound understanding of the issues related to the identification, assessment, diagnosis, treatment, and management of patients within your care; as well as working collaboratively with the multidisciplinary surgery team this maybe the job for you.
As an Advanced Nurse Practitioner, you will deliver high-quality, patient centred care while working autonomously within your professional scope. You will assess, diagnose, and manage patients, making clinical decisions in line with established procedures and protocols set out by our GPs.

Main duties of the job
Diagnosing and treating patients independently Prescribing medications safely and appropriately Requesting and interpreting investigations Delivering evidence-based interventions to optimise patient outcomes Working collaboratively with our multidisciplinary team to ensure excellent continuity of care. Maintain accurate, legible, and contemporaneous patient records. Support delivery of QOF, and other contractual requirements. Contribute to the development of practice policies and service improvements.

St Michaels is a training practice with 8,300 patients and are based across two sites on the south side of beautiful Bath, with ample parking available in each location. We are a friendly bunch with 5 Partners, 3 Salaried GP's, plus a very experienced nursing team, all supported by an experienced clerical and reception team. Work closely with GPs, nurses, pharmacists, social prescribers, and external services to ensure integrated patient care. Liaise with secondary care, community teams, and allied health professionals as needed. Participate in MDT meetings, case discussions, and safeguarding forums. Support the development of protocols, pathways, and best practice guidelines. To maintain a safe and therapeutic environment for all Primary Health Care Team staff, patients, carers and co-workers from other agencies. Ensure that all statutory regulations e.g. Health & Safety, COSHH, etc are implemented and adhered to. To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice Attend and participate in staff meetings
Person Specification
Experience Essential
Evidence of working autonomously and as part of a team. Experience of use of a medical software package (ideally TPP SystmOne, but training can be provided on this) Understanding of evidence-based practice. Proven ability to evaluate the safety and effectiveness of own clinical practice.
Desirable
Experience of working within a Primary Care setting. Experience in management long term conditions e.g. asthma, COPD, diabetes, CHD.
Qualifications Essential
Registered General Nurse/ or Paramedic. Currently registered with the NMC or HCPC with extensive post registration experience. Masters level qualification or equivalent in advanced clinical practice. Independent Non Medical prescriber. Current driving licence (as the role is based across the two sites)
Desirable
Current IRMER training and competency in requesting appropriate radiological investigations
