About this role

Job summary

The Medical Centre, Driffield, is looking for an Advanced Clinical Practitioner to work 3 (27 hours) or 4 (37 hours) days per week. Driffield is a market town in the East Riding of Yorkshire surrounded by the picturesque wolds, with easy links to nearby York and Beverley and is approximately 10 miles from the beautiful Yorkshire coastline.

The main surgery is located in Driffield town centre with a branch surgery in the village of Wetwang 6 miles west of Driffield. It is a semi-rural practice with 12500 patients.

We dispense to approximately 50% of our patients from Driffield and Wetwang.

Main duties of the job

We are looking for a highly motivated, enthusiastic, experienced ACP, who has clinical skills in general practice or urgent care. Our ideal candidate will have experience in a fast-paced environment , be confident in their clinical decision making and be able to deliver a high standard of safe clinical care.

They will demonstrate expert assessment and diagnostic skills and will commit to demonstrating critical thinking in the safe clinical decision-making process.

They will be able to evidence communication and collaborative working to meet the needs of patients and support the delivery of practice policies and procedures.

About us

The practice is a young, enthusiastic team which comprises of 2 partners, 6 salaried GPs and 4 Advanced Nurse Practitioners. We are a training practice with GP trainees from ST1-ST3.

Key duties

To work both in practice and across our care community, including in care homes, as an autonomous accountable ACP in the provision of a holistic approach to deliver quality patient services

The practice operates a Total Triage model of booking appointments. The Advanced Clinical Practitioner (ACP) will assess and triage patients on a rota basis, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition (as appropriate)

Undertake physical examinations as indicated by the patients condition

Advise patients on general healthcare and minor ailments and promote self-management where appropriate, including signposting patients to other community or voluntary services

Diagnose and manage both acute and chronic conditions

Work with patients in order to support compliance with and adherence to prescribed treatments

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health

Request, interpret and act on investigations

Identify red flags, underlying serious pathology and take appropriate action

Take a personalised care approach and population centred care approach to enable shared decision making with the presenting person

Carry out visits to patients homes and nursing/care homes as appropriate

Assist with the annual flu vaccination programme

Qualifications Essential

Qualified healthcare professional. Educated to Masters degree in the relevant area of expertise. Registered on your appropriate professional register such as NMC etc Qualified independent prescriber.
