Job description Job responsibilities
A clinical pharmacist in general practice plays a crucial role in improving patient care and medication management. Key responsibilities include:
Supporting patient care: Collaborating with clinical teams to provide best practices and mutual support.
Medication reviews: Conducting clinical medication reviews to manage patients with complex polypharmacy and chronic conditions.
Patient education: Providing clinical expertise and educational resources on medication sand treatment pathways.
Administrative support: Managing prescription queries, conducting audits, and ensuring safe prescribing practices.
Clinical leadership: Overseeing medicines optimization and quality improvement initiatives within the practice.
This role is pivotal for enhancing patient outcomes and operational efficiencies in general practice settings.
Person Specification
Experience Desirable
Experience of working in a GP practice preferred.
Qualifications Essential
Masters in Pharmacy Need to have completed Primary Care Pharmacy Education Pathway.
Desirable
Prescriber preferred, but not essential
