Job summary

An exciting opportunity has arisen to join our Community Intermediate Care Team in South East Essex. The current vacancy is within the Cumberlege Intermediate Care Centre (CICC).

CICC is a 22 bedded Intermediate Care Unit which currently provides stroke rehabilitation. We therefore require a skilled and experienced staff member in physical rehabilitation including assessing and treating patients with stroke/neurological diagnoses.

The successful candidate will work as part of a large and dynamic MDT, comprising of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Nurses, Healthcare Assistants and Therapy Assistants.

Main duties of the job

ROLE SUMMARY

The post holder is responsible for provision of Occupational Therapy assessment, treatment and evaluation of stroke/neurological patients and frailty patients in a community rehabilitation setting. To participate in multidisciplinary rehabilitation services within a defined area of our Intermediate Care Service. The multidisciplinary team includes Rehabilitation Assistants, Therapy Assistants, Physiotherapists, Occupational Therapists, Speech and Language Therapists and Nursing colleagues. To work autonomously to provide evidence based, high quality patient care and rehabilitation which is personalised to the individual's goals and needs. Deliver a compassionate, dignified and respectful service to patients at all times and ensuring that the values outlined in the NHS constitution are adhered to daily and any matters of concern are raised with the relevant Line Manager or through the necessary processes within the Trust. KEY RESPONSIBILITIES

General responsibilities

To provide expert knowledge and skills to support the multi-disciplinary team and facilitate effective assessment and rehabilitation of intermediate care patients (Stroke/Frailty/IMC) encouraging independence and recovery. To facilitate effective prevention of admission to hospital or residential care and facilitation of discharge from hospital to the Intermediate Care Services. Manage and co-ordinate a caseload of patients with complex conditions including unstable fractures, complex neurological, frailty and stroke conditions. To oversee Occupational Therapy provision and ensure that accurate records and risk assessment are maintained and regularly reviewed. To provide expert leadership to junior staff through support, supervision and appraisal. Work in close liaison with other professionals within Reablement, Social Services, Acute Hospitals and Integrated Discharge Teams to achieve and maintain smooth care pathways for patients. Work in conjunction with the Therapy Lead for Intermediate Care, Intermediate Care Matrons and Head of Intermediate Care on strategic development of services in line with future service demands. Assist in the collection of information to support the evaluation of services once commissioned.

Service Provision

Work collaboratively with all members of the team to ensure that an appropriate therapeutic environment is maintained that will support active rehabilitation programs for individual patients. Organise and maintain own diary and manage time effectively in order to meet the demands of the Intermediate Care Services, clinical caseload and supervision/support for other Occupational Therapy staff within your area. Participate in weekly MDT meetings and daily board rounds to review individual patient progress and organise onward referral to external services as required.

Contribute to service development to enhance decision making through offering operational advice. Take responsible care for the health and safety of self and others, ensuring policies and procedures for maintaining the provision of a safe environment are adhered to. Identify and report on practices and areas of work that may have risk management implications to ensure the safety of patients and colleagues, and ensure that all risk events are reported promptly and in co-ordination with the Service Manager.

Clinical Responsibilities

To apply specialist skills and knowledge in order to establish professional competence and fitness to practice as a Band 6 Occupational therapist To be responsible for the planning and delivery of occupational therapy within a defined area of our Intermediate Care Service, prioritising clinical needs to provide an effective service. To work with a caseload of patients with varying difficulties including complex health and social care needs. To independently carry out rehabilitation programmes aimed at improving or maintaining independence. To monitor, evaluate and modify interventions in order to measure progress and ensure effectiveness. To assess and order equipment as identified by patient need. To ensure all equipment provision is fit for purpose and patient/team members are aware of the instructions for safe use. To apply a high level of understanding of the effect of disability and provide advice to users/carers as appropriate. To respond effectively to changing needs of the patient caseload, managing the sometimes conflicting prioritises of prevention of admission and discharge. To make appropriate onward referrals to ensure identified patient needs continue to be met after discharge from Intermediate Care Services. To adhere to the COT Code of Ethics and Professional Conduct.

Professional Responsibilities:

To act as a resource to Intermediate Care staff offering specialist Occupational Therapy advice and help in all aspects of therapy. Attend case conferences as appropriate, giving specialist advice and input as required. Attend multidisciplinary meetings with other agencies involved in Intermediate Care including Social Service, housing, the acute Hospital and Long Term Providers, acting as the representative from Intermediate Care ensuring accurate and timely information is available to facilitate patient care. Ensure effective partnership working with other services, notably other Intermediate Care Services, Wider Community Health Services, GPs, Acute Hospitals and Social Services. To demonstrate ongoing personal development through participation in internal and external development opportunities, recording learning outcomes in a portfolio. To facilitate compliance of other therapy staff with local and national Standards of Practice. To exercise good practice with time management, punctuality and reliable attendance. Demonstrate ability to review and reflect on own practice and performance through effective use of professional and operational supervision and appraisal in line with local guidelines. To be involved in the supervision framework for therapy assistant staff and contribute to the organisation of the appraisal process. To engage actively in practice development, audit and research activities relevant to occupational therapy and/or the service area, to promote evidence based practice.

Managerial Responsibilities

To provide line management to Junior Occupational Therapists/therapy assistants within intermediate care services as necessary. Provide advice and support staff on clinical rehabilitation issues. Ensure effective staff management including adherence to policies and procedures to safeguard the well-being of patients and staff, to include staff appraisal and supervision, mandatory training, probationary and sickness policies as necessary. To allocate work to junior therapists, therapy assistant or care staff as necessary. To assist in the prioritisation and acceptance of referrals to the team in a timely way. Ensure the economic and efficient use of all resources. To develop and maintain effective professional communication structures within the team. Ensure all junior staff are managed in accordance with good personnel practice with particular attention to induction, appraisal and supervision. To establish communication networks with other team members, patients, carers and other health, and social care agencies. To undertake joint visits with other professional outside of intermediate care services as required. To support /lead the discharge planning process for patients on the case-load. Toensure written and electronic records and activity data are maintained, in accordance with Professional and Organisational Standards. To work effectively as a member of the multidisciplinary team to support the planning and achievement of local service objectives. To support the management team and contribute to the delivery of the Trusts Development plan for Intermediate care. To be involved in the identification of service development needs and supporting the Service Manager with changes in service provision as necessary.

ADDITIONAL DUTIES

Complete mandatory training in line with Trust policy and procedures

To participate in the staff appraisal process and to undertake for any staff you manage

To keep yourself updated on all matters relating to Trust policy

To provide management supervision where appropriate

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts, where appropriate and operationally required.
