Job summary
You will be responsible for assisting with the delivery of patient centred care under the supervision of a social worker, providing practical support to service users in performing day to day activities, guidance and education, and in assisting in their management and care based upon individual needs under the guidance of a senior member of the Team.

Main duties of the job
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, working shifts where appropriate and operationally required.
In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.

The successful candidate will work with carers and families of people with mental health needs under the direction of a Consultant Social Worker. You will support and enable the carers to connect with local assets within the communities to help build resilience.

Person Specification
Education/Qualifications Essential
NVQ Level 3 Health or Social Care or equivalent. Good written and spoken English
Knowledge Essential
Understanding of working with diverse populations, and diverse backgrounds Understanding carers rights under the Care Act , 2014 Understanding of Equality Act, 2010 Basic understanding of the nature of mental health conditions and how it can affect people's day to day life. Understanding of working within a multi-disciplinary health, social care in a community setting.
Desirable
Knowledge of the Children's Act Knowledge of the impact of social determinants of health and wellbeing Knowledge of issues affecting specific groups of carers - e.g. young carers, parent carers, dual diagnoses, etc.
