Clinical Team Lead - 22.5 hours part time/Job shareTo provide clinical leadership and clinical advice to staff working within the CLDT's and to provide clinical leadership within the CLDT. To support nurses in complex case management and ensure that there is provision of appropriate clinical and managerial supervision within the Community Learning Disability Team.To ensure there is accountability regarding the community caseload and current waiting list. To liaise closely with Clinical Nurse Specialists to provide seamless service across the Specialist LDdirectorate.
Main duties of the job
The post holder will support and oversee operational delivery of the teams and will work closely with the Business Support Manager to ensure that risk registers, contractual information and KPI's are up to date and meeting set standards. They will be required to work autonomously in order to address issues impacting service delivery with overall support and strategic direction from the Head of Service.

Job description Job responsibilities
KEY DUTIES AND RESPONSIBILITIES:
1. Provide clinical leadership and clinical advice providing a visible point of call for staff to go to regarding clinical and operational issues.2. To provide supervision to members of the community learning disability team.3. To ensure there is accountability regarding the community caseload and current waiting list.4. To help define the role and parameters around community nursing.5. To support the team in complex case management.6. To provide a constructive clinical lead to ensure that MDT decision making is in line with clinical.7. Processes i.e. Duty and Intake ensuring all service users referred have a health coordinator.8. To manage clinical and operational performance issues for the CLDT and work supportively with other Professional leads cross the Directorate to ensure a seamless service.9. To assist the team to implement clinical processes in line with the Clinical Strategy.10. To lead on clinical audit and service improvement as required for the CLDT.11. Working closely with the Head of Nursing and other professionals leads to address and progress any issues highlighted.12. To ensure the quality of care received by people with learning disabilities is appropriate to meet their needs, highlighting gaps where necessary.

Person Specification
Qualifications & Training Essential
Qualified Nursing Practitioner RNLD. Or equivalent professional registration. Degree or equivalent experience in relevant subject.
Experience Essential
Significant experience at working at Band 6 or above. Experience of working with statutory, voluntary or private sector organisations including direct working with local authority colleagues. Experience in working in areas which include: o Assessing clients with complex health care needs. oEvidence of personal development of staff and staff appraisal.
Skills, Knowledge & Abilities Essential
The application of care models and theories into practice. Ability to identify deficiencies in services and initiate development into those areas. In-depth knowledge of current national agenda relating to the NHS, social care sector, learning disabilities and mental health. Knowledge of Mental Health, Physical Health. Positive Behaviour Support and Applied Behaviour Analysis. Demonstration of skills required to deliver clinical supervision. Knowledge of DOLS, Mental Capacity and Mental Health Act legislation.
Personal Attributes Essential
Ability to practice in an ethical way which considers the needs of the person receiving care and their best interests. Excellent communication awareness and skills. Ability to present information in a clear and concise way.
