Job summary
We have an exciting opportunity for a Registered Nurse Band 5,to join our Swindon Hospital at Home service. The service is currently undergoing an exciting period of transformation, making this an excellent time to become part of a dynamic and innovative team. Post will be based at the Swindon Intermediate Care Centre (SwICC).
As a Registered Nurse, you will support admission avoidance by working across acute and community services to provide multidisciplinary, patient centred care in a home setting. You will join a compassionate and committed team dedicated to delivering safe, personalised care and making a meaningful difference to patient outcomes outside the traditional hospital environment.

Main duties of the job
As a Registered Nurse, you will work as part of a multidisciplinary team, collaborating closely with Nursing, Therapies, and Care Coordination colleagues to support the delivery and ongoing development of the Hospital at Home service. With support from senior clinicians, you will manage your own clinical caseload and deliver care in community settings, helping to avoid unnecessary hospital admissions and support timely discharges through partnership working with BaNES, Swindon and Wiltshire (BSW) services.
You will be responsible for coordinating specialist, personcentred care for patients with urgent health needs, including clinical assessment, care planning, recognition of deterioration, escalation, and accurate documentation. The role also involves day today caseload management, acting as a patient advocate, supporting colleagues and learners, and contributing to continuous improvement through reflective practice and a Just Culture approach.
