Job summary
We are seeking an experienced and motivated Clinical Pharmacist with Independent Prescribing (IP) qualification to join our established multidisciplinary primary care team. This is an excellent opportunity to work within a supportive and well-developed pharmacy service, contributing directly to high quality, patient-centred care.
You will work alongside an established pharmacy team, including two experienced pharmacy technicians and a dedicated prescription team, allowing you to focus on clinical decision-making, optimisation of medicines, and long-term condition management.
Main duties of the job
The post holder will play a key role in delivering safe, effective, and evidencebased medicines management services within primary care.
Key Responsibilities:
Provide clinical pharmacy support to patients and the wider primary care team Independently prescribe and manage medicines for patients within agreed clinical areas Manage long term conditions in line with local and national guidelines Support medicines optimisation, deprescribing, and cost effective prescribing Support Pharmacy Technicians with medicine reconciliations following hospital discharge and outpatient clinic letters Contribute to prescribing audits, QOF, and quality improvement projects Work collaboratively with GPs, nurses, pharmacy technicians, and administrative teams Support training and development within the pharmacy and wider practice team

Haslemere Health Centre is alarge, friendly, and enthusiastic GP practice set in the beautiful Surrey Hills. Our multidisciplinary team includes GP Partners, Salaried GPs, Nurses, Healthcare Assistants, Pharmacist, Pharmacy Technicians, Physiotherapist, social prescriber, and visiting paramedic.
We are atraining practice, regularly hosting GP registrars and FY2 doctors. Two of our GPs also run the local GP registrar training scheme.
Our main site is amodern single storey NHS Health Centreon the grounds of Haslemere Hospital, with20 consulting rooms, abranch surgery in Fernhurst, and dedicated admin space within the hospital.
We are part of theWest of Waverley PCNandProcare GP Federation.

Experience working as a clinical pharmacist in primary care or GP settings Strong clinical knowledge of long term conditions and medicines optimisation Experience managing lipid modification / cholesterol (e.g. statins, ezetimibe, newer lipid lowering therapies) Competent use of clinical systems (e.g. EMIS or willingness to learn)
