Job summary
The post holder will be an experienced healthcare practitioner who, acting with in their professional boundaries, will provide care Cross-site at both Epsom and St Helier Hospitals, for patients presenting in the Emergency Department. From initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within the department.
The post holder will demonstrate critical thinking in the clinical decision making process. They will work collaboratively within their MDT to meet the needs of the patients, supporting the delivery of policy and procedures, and providing professional leadership as required. In order to work at this level, professional body e.g. NMC requirements for advanced practice must be met.
Main duties of the job
Main Responsibilities and Duties?Assess individuals, families and carers, holistically using a range of differentmethods, such as physical assessment and history taking, ordering andinterpreting diagnostic tests or advanced health needs assessments.?Use your expert knowledge and clinical judgement to decide whether to referpatients for investigations and make diagnoses?Communicate effectively with patients, carers and the multidisciplinary team?Decide on and carry out treatment, which may include the prescribing ofmedicines, or referring patients to an appropriate specialist.?Ensure the provision of continuity of care.?Assess and evaluate with patients and / or carers, the effectiveness of thetreatment and care provided and make changes as needed.?Work autonomously within a wider health care team and as a lonepractitioner
