About this role

Job summary The IOS team exists because people in acute mental health distress often fall between servicestoo unwell to cope alone but not always needing long hospital stays. IOS bridges this gap by providing intensive, practical, and emotional support at home to keep people safe, stable, and moving forward.

Main duties of the job Were recruiting a Recovery Navigator to join our Intensive Outreach Service in BANES, supporting people referred from AWP services for up to six weeks to help prevent hospital admission or to support their independence after leaving a psychiatric hospital, as well as helping those experiencing serious or sudden mental health difficulties.

Job description Job responsibilities Ensure the service is delivered in a warm and welcoming way, demonstrating ambition and hope for all service users. Build trusting and collaborative relationships with service users.

Uphold the Mental Health and Wellbeing Partnership values of focusing on recovery, valuing equity & integrity, doing what works, sharing learning, and leading change and transformation.

Work as part of an integrative partnership to provide a joined-up offer to individuals accessing the service. You will liaise with colleagues and build a strong working partnership with other agencies such as GPs, Health and Social Care, other Voluntary and Community Services to coordinate referrals, to take initiative to share learning new ideas and trends, and deliver the service users support package.

Attend and contribute in a positive and proactive way to daily handovers and Multi-Disciplinary Discussions.

Work with individuals through trusting relationships to collaboratively develop and review person-centred support plans that focus on their goals and hopes, using DIALOG+ support tool and evidence-based interventions. Working alongside AWP, PCLS and other professionals to identify appropriate pathways of support to achieve these using Your Team, Your Conversation, Your Plan assessments and other self-management tools.

Person Specification

Experience Essential

A minimum of two years significant experience working with people with mental health needs, or people with complex needs Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions) Proven experience of carrying out needs and/or risk assessments and the support planning process Experience of working with a number of individuals with competing needs and priorities

Desirable

Proven experience of mental health issues and/or services, either as a user, carer or supporter.
