Job summary
An exciting opportunity has arisen for an experienced Band 3 Healthcare Assistant to join the Merton District Nursing Service.
Working as part of the wider community nursing team, you will support the delivery of high-quality, safe and personalised care to housebound patients in their own homes. You will work alongside registered nurses and other healthcare professionals, undertaking delegated clinical and administrative duties within your level of competence.
We are looking for a compassionate, motivated and reliable individual with excellent communication skills who is committed to delivering effective, patient-centred care.

Main duties of the job
Your main duties will include delivering a range of delegated clinical interventions and providing holistic, person-centred care to housebound patients in their own homes. You will work under the direction of registered nurses and use a range of clinical skills within your level of competence.
You will need excellent communication and organisational skills, the ability to work effectively as part of a team, and the flexibility to respond to the changing needs of the service. You should remain calm under pressure and have good IT skills to support accurate and timely electronic clinical documentation.
The Merton District Nursing Service provides care to patients with a wide range of health needs within the community. The service works closely with GPs, hospitals, hospices, social care and other healthcare professionals to provide coordinated care and support patients to remain safely at home.
You will be fully supported by the wider District Nursing team, with opportunities for ongoing learning, competency development and continued professional development.

Person Specification
Qualifications Essential
Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or a recognised equivalent course Willingness to undertake further training as appropriate
Desirable
GCSE grade 9 - 4 or equivalent in English and Maths Registration with the national regulatory body (when it becomes available).
Experience Essential
Experience of working in a health or social care setting Experience with the use of Microsoft Office and a variety of databases.
Desirable
Experience of working as part of a team Experience with the NHS patient information database (Rio, CIS)
Skills & Knowledge Essential
Knowledge of NHS Knowledge of multi-agency services available and processes for referral Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being An awareness of risk management

Previous experience of working on an electronic record system
