Job summary
The Complex Care managers operate as clinical case managers to ensure individual patients commissioned services meet their needs, are safe and deliver value for money. Through the understanding of complex care policies, regulations, and best practices, maintain a high qualityservice.
These roles are vital in the delivery and management of placements and support to our mostvulnerable clients. The team look after all clients including Adults with complex Mental Health, complex Children and Young People, Adults with Learning Disabilities and Autistic Adults.
Main duties of the job
o To review placements and packages of care funded by the ICB as required prioritising high risk and high cost placements;o Manage and financially contribute to S117 and Joint Funding Panels/decisions with the Local Authorities using locally agreed criteria;o Support the management and reviews of mental health rehab hospital placements and discharges for Complex CYP requiring additional support above local provision;o Discharge- support placement flow attending DTOC, MADE events, LAEP and escalations as required;o Undertake brokerage on behalf of LBHF if required;o Manage quality issues in regard to placements, safeguarding, provider concerns and liaising with CQC;o Undertake dispute resolution and where required commission legal support related to DoLs/S117/MHA -i.e. judicial review, court of protection and responsible commissioner disputes;o Maintain accurate case records and financial records on Caretrack systems;

Education/Qualification Essential
Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area Post-graduate degree in Management Studies or equivalent
