About this role
Job summary Working with the District Nursing team, you'll provide high-quality care in people's homes so that they can receive safe, effective support and maintain their independence. Based on the role outlined inthis position plays an important part in supporting patients with a wide range of health needs and ensuring care is tailored to their individual circumstances. You'll work closely with patients, families, carers and a range of health and care professionals to assess needs, plan care and deliver evidence-based nursing interventions. The role offers opportunities to develop your skills, work autonomously and contribute to service improvement within a supportive multidisciplinary team.
Main duties of the job What you'll do:
