Job summary As part of the Mental Health and Wellbeing Partnership in Bath and Northeast Somerset, Swindon and Wiltshire (BSW), Second Step delivers community mental health support in collaboration with Alabar, Missing Link and Nilaari. Together, were transforming mental health services by focusing on trauma-informed, person-centered, and culturally inclusive approaches.
Main duties of the job We are recruiting a Senior Physical Health Check Coordinator to join our Access Community Mental Health Team in Swindon to develop effective and trusting relationships with individuals who have severe mental health conditions such as bipolar, schizophrenia or psychosis, in order to support them to access SMI Physical Health Checks and support them to identify appropriate services and support to improve their physical health.

Job description Job responsibilities Speak to individuals to who are on the SMI (Severe Mental Illness) register, describe the benefits and what happens at a Physical Health Check and enable their appointment.
Develop a rapport with individuals and identify barriers to accessing services and support to attend local services and interventions that could improve their physical health.
Contact individuals after their Physical Health Check, discuss the results and any recommendations for improving their physical health.
Work autonomously across the Locality to implement the service and drive its success.
Build relationships with stakeholders, particularly Primary Care Networks, increase awareness of the service and codesign processes across the Locality.
Promote recovery, inclusion, and equity in everything you do.
Person Specification
Experience Essential
A minimum of one year experience working with people with mental health needs, or people with complex needs. Proven experience of identifying risk, effective documentation and escalation of risk concerns. Experience of working with a number of individuals with competing needs and priorities.
Desirable
Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions and offending). Proven experience of mental health issues and/or services, either as a user, carer or supporter.
Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
