Job summary As a Specialist Public Health Nurse - School Nurse, the post holder will work within an integrated neighbourhood team contributing to and supporting the delivery of the Healthy Child Programme 5 - 19 services.
Working within Healthy Together, the post holder will contribute to improving access, identifying health needs, improving outcomes and reducing health inequalities by providing a comprehensive neighbourhood based service to meet the needs of local children, young people and their families. Through a whole family approach, the focus will be on prevention of ill health, promotion of positive health and well-being and working collaboratively with the wider community.
As a member of the Healthy Together neighbourhood team the post holder will be responsible for taking an active role with the 5 - 19 service in the delivery of integrated health services. Training and the competency framework will support all areas of work.

Main duties of the job As a Public Health Nurse (School Nurse), you will provide professional nursing leadership, clinical advice and evidence-based interventions to improve health outcomes for children, young people and families. You will assess health and social needs, develop care plans, evaluate outcomes and identify safeguarding concerns, taking appropriate action in line with Trust and safeguarding partnership policies. The role involves identifying emerging health needs, promoting health and wellbeing, and applying public health nursing knowledge to support individuals, families and communities. You will work effectively in complex situations, using sound judgement, prioritising workload and responding flexibly to urgent needs. You will contribute to and lead delivery of the Healthy Child Programme in line with Standard Operating Guidance, signposting families to appropriate services and empowering them to access support. Care will be delivered across a range of settings to meet diverse needs. You will manage your own caseload, maintain accurate records, communicate effectively with families and partner agencies, and produce high-quality reports and referrals. The role requires sharing complex and sensitive information, including safeguarding concerns, and using interpreters or health advocates where required. You will support effective clinical leadership, supervise practice to maintain quality standards, and act as a role model by mentoring, teaching and supporting students & learners.
