Job summary
Calling all Registered Nurses (Adult, Mental Health or LD) or Allied Health Professionals registered with the HCPC who have an interest in supporting individuals who have an Acquired Brain Injury and/or Complex care needs. The Complex Case Senior Practitioner role is part of the small and friendly Complex Care team that sits alongside the All Age Continuing Care Team in the cluster ICB of NHS Dorset, NHS Somerset, NHS Bath, NE Somerset, Swindon & Wiltshire. The role operates Monday to Friday core hours, with no night shifts or weekends, with a hybrid approach of office work, remote working and face to face assessments with individuals across the cluster footprint. Main duties of the job
The purpose of the role is to ensure that those requiring complex rehabilitation and care are assessed appropriately and accepted onto an NHS funded pathway. Practitioners will undertake review following MDT recommendation to allow for a consideration of entry onto the funded pathways and will undertake reviews on patients who are already in receipt of funding for complex care and Acquired Brain Injury rehabilitation across the whole cluster region.

To provide and receive highly complex, sensitive and contentious information, communicating unwelcome decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism.To review and reassess at agreed intervals the progress being made by the individual alongside the MDT, the individual and their representatives.To have consideration for the principles of the Mental Capacity Act and to work within them.To review any service and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract.Sign posting to relevant teams and organizations.Responsible for oversight /assurance of commissioned equipment that may be required by the cohort.To plan with the MDT an appropriate exit from the funding stream including the completion of CHC checklist where this is appropriate.To ensure that person centred quality care and support is at the forefront.Work with system partners ensuring best practice is adhered to in respect of all commissioned services that support the needs of the complex/ABI/neurorehabilitation caseload.Support and provide recommendations for funding arrangements for individuals, working with multi-disciplinary teams and relevant panels as necessary.Be a link between key stakeholders to promote collaborative working and provide a seamless service for patients.Required to maintain constructive relationships with a broad range of internal and external stakeholders, including acute hospitals, care providers, community health providers, voluntary sector organisations and local authorities.Participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams. Work with members of the team to develop and implement data collection systems that will provide accurate and timely data.Communicate information, risks, issues and dependencies, including briefings and reports to teams and a range of internal and external colleagues.We work as a team of teams, recognising that improving health and care depends on strong collaboration across traditional boundaries. Qualifications Essential
Please also see attached Person Specification. Relevant professional registration. Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area. Further training or significant experience in project management, financial management or supporting change management processes. Comprehensive knowledge of project management and/or health information systems development. Workforce development knowledge and experience. A good understanding of the health and social care environment and roles and responsibilities within it.

Skills for managing projects ensuring they meet financial targets. Ability to work on own initiative and organise own workload without supervision working to tight and often changing timescales. Previous experience in project management and planning. Experience and understanding of evaluating and measuring the performance of health services. Experience in communications and stakeholder management. Previous experience in similar role in public sector.
