Job summary
Palliative and End of Life Care Services provide various of levels of support to people with limited conditions and those who are in the final phase of life. The service provides, one to one support as well as advisor capacity to other professionals. The service will work through a central point of referral, working in conjunction with the Community Nursing Teams.
The Palliative Care Team is a health and social care team that provides an expert clinical and social care service for patients with challenging and complex life limiting diagnoses and their families. All senior team members act as clinical or social care experts, leaders and educators for their service areas. The team provides a service in all care settings across the North East Lincolnshire locality.

The role plays a critical part in ensuring Palliative Care Services are delivered safely, effectively and in line with local and national standards, while supporting our strategic ambitions and continuous growth. Working closely with senior leaders, commissioners and partner organisations, you will work alongside the Palliative Care Consultant to provide specialist care to complex palliative patients, support team management and help shape and deliver services that reflect our commitment to high quality care, coproduction and our Commercially Compassionate ethos.
You will demonstrate advanced clinical skills across the four pillars of advanced practice: Clinical practice, leadership and management, education and research working in various care settings include community, hospital and care homes as appropriate
You must be Registered with the NMC/HCPC and have a Masters degree or equivalent in Advanced Clinical Practice as well as being an independent prescriber.
