Job summary Caseload Coordinator - Urgent Community Response & Virtual Ward: Permanent (30 hours)

This position is for 1 part time 30 hours per week post, some weekend working is required.

This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.

UCR/HaH is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis. The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.

This role requires a highly organised person, with ability to prioritise their workload and work in a calm and effective manner and communicate well at all levels.

Please note that whilst the trust does offer sponsorship this role is unfortunately not eligible for visa sponsorship under the UK Health and Care Worker/Skilled visa route. This is due to the position not meeting the minimum salary threshold of £25,000 per annum and/or the required skill level set by UK Visas and Immigration.

Main duties of the job

This role will involve managing the case load of clinical staff within the Urgent Community Response (UCR) and Hospital at Home (HaH) Team. It will involve gathering, inputting and providing essential data for patients, helping to maintain accurate records and enhancing the team's ability to provide personal care.

The role is responsible for the administration support and caseload co-ordination within the team to support the delivery of care to patients. The post holder would also be required to answer the telephone, which may include calls from patients, and information then passed onto a clinician.

Job description Job responsibilities

Ensure that the team case load is kept up to date. To participate and co-operate with the Multi Disciplinary Team to contribute to the effectiveness of the service as a whole. To manage and maintain the teams clinical caseload, including all associated administration tasks. To use multiple patient information management systems (EMIS, Docobo, Patient Centre etc.) to enter such information as required on the computerised patient information system. To use discretion, sensitivity and deal with information in a confidential and professional manner at all times. Oversee and monitor the distribution/collection of equipment that is used to monitor patients in their own home (tech-enabled care). Maintain an accurate database of the home monitoring devices. To be responsible for data collection, based on the outcomes from the clinical caseload. To participate in and organise team audits when appropriate.

If successful this vacancy will require an standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Person Specification

Qualifications Essential

Good general level of education, including English and Maths or equivalent qualifications or equivalent experience NVQ Level 3 or above in a relevant subject such as Customer Care, IT, or Business Administration or equivalent experience.

Knowledge Essential

To have an understanding of confidentiality Intermediate IT Skills including Microsoft Outlook, Word and Excel

Desirable

To have an understanding of current issues affecting the NHS and Social Care. Knowledge of rota systems Knowledge of Electronic Patient Record Systems

Skills and Abilities Essential

Excellent customer service skills with an excellent telephone manner alongside the ability to communicate clearly and accurately. Ability to work without direct supervision and to organise time effectively prioritising tasks. The ability to be adaptable to new and changing circumstances. Be able to demonstrate tact and diplomacy when working with others. Ability to work and problem solve under pressure whilst achieving targets Ability to maintain concentration with frequent interruptions from both staff and users of the service Demonstrate a commitment to achieving high quality standards of work Accurate and methodical in all aspects of work Ability to use initiative and meet deadlines Ability to build positive working relationships with colleagues internally and externally and work well within a team

Experience Essential

Experience of working as a team and independently Experience of working in a pressurised environment Relevant experience of providing a comprehensive clerical / administrative service Familiar with office procedures and deadlines

Desirable

Previous experience of a co-ordinating job role Experience of a variety of computer based programmes.
