Job summary Children's Community Nurse: band 6 - Permanent (30 hours)
We are looking for an experienced and motivated Children's Community Nurse to join our Children's Community Nursing Team.
This is an exciting opportunity to work as an autonomous practitioner, providing high-quality, evidence-based nursing care to children and young people with acute, chronic, complex and palliative care needs within their own homes and community settings.
The successful candidate will work closely with Paediatric Same Day Emergency Care (PSDEC), inpatient wards, Emergency Department, General Practice and the wider multidisciplinary team to support admission avoidance, facilitated discharge and care closer to home.
The role also provides opportunities to contribute to service development, quality improvement, pathway redesign and audit, supporting the continued development of integrated acute and community services.
We are looking for a confident, compassionate and proactive nurse with excellent clinical, communication and organisational skills, who is able to work effectively under pressure and across professional and organisational boundaries.

Main duties of the job
The post holder will work as an autonomous Children's Community Nurse, providing high-quality, evidence-based nursing care to children and young people with acute, chronic, complex and palliative care needs within their own homes and community settings.
Undertake comprehensive assessment, planning, implementation and evaluation of care. Manage a varied community caseload, prioritising changing clinical needs and responding appropriately to deterioration. Provide specialist nursing interventions and support children and families to manage complex and long-term conditions at home. Assess and triage referrals from PSDEC, inpatient wards, primary care and other services to determine suitability for community management. Support admission avoidance, facilitated discharge and Virtual Ward pathways, working closely with acute and community colleagues. Undertake home visits and virtual reviews, providing timely community intervention where clinically appropriate. Participate in daily clinical handovers and multidisciplinary discussions to identify children who may benefit from community nursing intervention. Work collaboratively with children, families and the wider multidisciplinary and multi-agency team. Recognise and respond to safeguarding concerns and escalate clinical concerns appropriately. Support and supervise junior staff and students and contribute to teaching and education.
